摘 要:目的:分析中性粒细胞与淋巴细胞比值(NLR)与小儿支气管肺炎细胞免疫水平相关性。方法:选择2018年2月—2020年2月信阳市中医院收治的56例支气管肺炎患儿作为研究组,将研究组分为普通组(普通支气管肺炎)30例和重症组(重症支气管肺炎)26例,选择同期在信阳市中医院体检的正常儿童60例作为对照组。观察研究组和对照组的NLR、细胞免疫水平、重症组和普通组的NLR值以及细胞免疫水平。结果:研究组的NLR值比对照组高,差异有统计学意义(t=9.003,P<0.05)。研究组的血清CD3^(+)、CD3^(-)CD19^(+)、CD19^(+)CD23^(+)、CD3^(+)CD4^(+)、CD4^(+)/CD8^(+)、CD3^(-)CD(16^(+)56)^(+)均比对照组高,差异有统计学意义(t=3.393、4.606、6.296、3.654、2.978、4.349,P<0.05),研究组和对照组的血清CD3^(+)CD8^(+)水平对比,差异无统计学意义(t=0.321,P>0.05)。重症组的血清CD3^(+)、CD3^(-)CD19^(+)、CD19^(+)CD23^(+)、CD3^(+)CD8^(+)、CD3^(+)CD4^(+)、NLR水平均比普通组高,差异有统计学意义(t=6.192,6.703,7.039,4.393,4.325,3.867,P<0.05);重症组和普通组的血清CD3^(-)CD(16^(+)56)^(+)水平对比,差异无统计学意义(t=0.614,P>0.05);重症组的血清CD4^(+)/CD8^(+)水平比普通组低,差异有统计学意义(t=4.325,P<0.05)。采用Pearson分析发现,研究组患儿的NLR值与血清CD19^(+)CD23^(+)呈正相关,差异有统计学意义(r=0.236,P<0.05),与CD4^(+)/CD8^(+)、CD3^(+)CD4^(+)呈负相关,差异有统计学意义(r=-0.382,-0.217,P<0.05)。结论:NLR与支气管肺炎患儿免疫功能相关,NLR检测临床上可用于辅助评价支气管肺炎患儿的免疫功能。Objective:To analyze the correlation between neutrophil-to-lymphocyte ratio(NLR)and cellular immunity in children with bronchopneumonia.Methods:56 children with bronchial pneumonia admitted to the hospital from February 2018 to February 2020 were selected as the study group,and the study group was divided into the general group(common bronchopneumonia)with 30 cases and the severe group(severe bronchopneumonia)with 26 cases.60 children with normal physical examination in the hospital during the same period were selected as the control group.The NLR and cellular immunity levels of the study group and the control group,the NLR values and cellular immunity levels of the severe group and the normal group were observed.Results:The NLR value of the study group was higher than that of the control group,and the difference was statistically significant(t=9.003,P<0.05).The serum CD3^(+),CD3^(-)CD19^(+),CD19^(+)CD23^(+),CD3^(+)CD4^(+),CD4^(+)/CD8^(+),CD3^(-)CD(16^(+)56)^(+)in the study group were higher than those in the control group,and the difference was statistically significant(t=3.393,4.606,6.296,3.654,2.978,4.349,P<0.05).There was no statistically significant difference in serum CD3^(+)CD8^(+)levels between the study group and the control group(t=0.321,P>0.05).Serum CD3^(+),CD3^(-)CD19^(+),CD19^(+)CD23^(+),CD3^(+)CD8^(+),CD3^(+)CD4^(+),NLR levels in the severe group were higher than those in the normal group,the difference was statistically significant(t=6.192,6.703,7.039,4.393,4.325,3.867,P<0.05).There was no statistically significant difference in serum CD3^(-)CD(16^(+)56)^(+)levels between the severe group and the normal group(t=0.614,P>0.05).Serum CD4^(+)/CD8^(+)levels in the severe group were lower than those in the normal group,and the difference was statistically significant(t=2.234,P<0.05).Pearson analysis found that the NLR value of the children in the study group was positively correlated with serum CD19^(+)CD23^(+)(r=0.236,P<0.05),and negatively correlated with CD4^(+)/CD8^(+),CD3^(+)CD4^(+),the differennce
