大接经针法联合小针刀治疗中风后偏瘫肩痛的临床研究

摘　　要：目的:探讨大接经针法联合小针刀治疗中风后偏瘫肩痛的疗效。方法:随机将本院收治的74例中风后偏瘫肩痛患者分为对照组37例(小针刀疗法)和治疗组37例(大接经针法联合小针刀疗法),对比治疗后的临床疗效、VAS评分、肩关节活动度、改良Barthel指数评分、上肢Fugl-Meyer评分、SHSS评分及血清CRP、NO水平。结果:治疗后治疗组总有效率显著较高(P<0.05);两组上肢后伸、前屈、内旋、外展及外旋等角度均明显增加(P<0.05)。并且治疗组增加较多(P<0.05);两组治疗前VAS评分、改良Barthel指数评分和上肢Fugl-Meyer评分差异无统计学意义(P>0.05);治疗后两组VAS评分显著降低,改良Barthel指数评分和上肢Fugl-Meyer评分显著升高(P<0.05),并且治疗组改善较多(P<0.05);两组SHSS评分及血清CRP、NO水平显著降低(P<0.05)并且治疗组降低较明显(P<0.05)。结论:中风后偏瘫肩痛患者采用大接经针法联合小针刀治疗具有较好的疗效。Objective: To explore the curative effect of whole meridians connection acupuncture combined with acupotomy in the treatment of post-stroke hemiplegia and shoulder pain. Methods: 74 patients with post-stroke hemiplegia and shoulder pain were randomly divided into the control group(n=37) and the treatment group(n=37). The control group was treated with acupotomy;on which basis, the treatment group was also treated with whole meridians connection acupuncture. Clinical efficacy, VAS score, motion range of shoulder, modified Barthel index score, upper limb Fugl-Meyer score, SHSS score and serum levels of CRP and NO were compared between the two groups after the treatment. Results: The total effective rate of the treatment group was significantly higher than that of the control group(P<0.05). The angles of extension, flexion, pronation, abduction and extorsion of upper limbs were significantly increased in both groups(P<0.05);of which the increases were more significant in the treatment group(P<0.05). There were no statistical differences in VAS score, modified Barthel index score and upper limb Fugl-Meyer score between the two groups before the treatment(P>0.05);VAS score was significantly decreased, and modified Barthel index score and upper limb Fugl-Meyer score were significantly increased in the two groups after the treatment(P<0.05), of which the improvements were more significant in the treatment group(P<0.05). SHSS score and serum levels of CRP and NO were significantly decreased in the two groups after the treatment(P<0.05), and the decreases were more significant in the treatment group(P<0.05). Conclusion: Whole meridians connection acupuncture combined with acupotomy has better curative effect on post-stroke hemiplegia and shoulder pain.
