超短波联合超声引导下芒针治疗腰椎间盘突出症临床研究

摘　　要：目的:观察超短波联合超声引导下芒针针刺腰丛以及坐骨神经干治疗腰椎间盘突出症的临床疗效。方法:将80例腰椎间盘突出症患者随机分为两组,其中治疗组40例、对照组40例。治疗组采用超声引导下芒针针刺腰丛以及坐骨神经干加超短波疗法,对照组采用超短波疗法,每天治疗1次,共治疗3周。分别在治疗前后比较治疗组和对照组两组患者的VAS评分、JOA评分、JOA评分改善率、Oswestry评分以及有效率。结果:采用重复测量方差分析,判断不同治疗方案随时间变化对结果的影响。VAS评分和Oswestry评分的结果显示,超短波联合超声引导下芒针治疗的主效应在统计上均显著(P<0.05)。时间因素对腰椎间盘突出症患者疗效影响差异有统计学意义(P<0.05),即随着治疗时间的延长,治疗组的疗效作用增强。JOA评分的结果显示,在治疗后,治疗组与对照组患者的差异有统计学意义(P<0.05),并且治疗组的时间因素存在单独效应。JOA评分改善率结果显示治疗组的改善率要优于对照组,疗效差异具有统计学意义(P<0.05)。结论:超短波配合超声引导下芒针针刺腰丛以及坐骨神经干治疗腰椎间盘突出症能迅速缓解症状,提高患者日常生活能力,效果好、精度高,值得临床推广。Objective: To observe the clinical effect of ultrashort wave combined with ultrasound-guided elongated needling lumbar plexus and sciatic nerve trunk in the treatment of lumbar disc herniation(LDH). Methods: 80 patients diagnosed with LDH were randomly divided into the treatment group(n=40) and the control group(n=40). The control group was treated with ultrashort wave therapy, once a day for a total of 3 weeks;on which basis, the treatment group was also treated with ultrasound-guided elongated needling lumbar plexus and sciatic nerve trunk. VAS score, JOA score, JOA score improvement rate, Oswestry score and effective rate were compared between the two groups before and after the treatment. Results: Repeated measures analysis of variance was used to investigate the impact of different treatment options on the results over time. The results of VAS score and Oswestry score showed that the main effects of ultrashort wave combined with ultrasound-guided elongated needling were statistically significant(P<0.05). The time factor had statistically significant influence to the efficacy of LDH(P<0.05), as the treatment time increased, the therapeutic effect became stronger in the treatment group. The results of JOA score showed that after the treatment, the difference was statistically significant between the treatment group and the control group(P<0.05), and the time factor had significant effect in the treatment group. The results of JOA score improvement rate showed that it was better in the treatment group than that in the control group, and the difference in efficacy was statistically significant(P<0.05). Conclusion: Ultrashort wave combined with ultrasound-guided elongated needling lumbar plexus and sciatic nerve trunk can quickly relieve symptoms and improve the patients’ ability of daily living. It has good effect and high accuracy, and it is worthy of clinical promotion.
