摘 要:目的探讨超声心动图指导下的心脏听诊在优化医学生心脏体格检查听诊中的意义。方法选择临床医学专业270名医学生:150名参加超声心动图指导下心脏听诊教学,分为先听诊组(75名)和后听诊组(75名);常规听诊组为120名医学生,均完成心脏听诊部分教学,通过理论考试、实践操作技能考核对学生进行听诊考核,并完成课堂教学调查问卷。结果(1)超声心动图指导下心脏听诊组在发挥学习主动性、突出重点内容、增长临床实践能力等方面更能得到学生认可,较常规临床听诊组差异均有统计学意义(P<0.05);超声心动图指导下先听诊组在以上方面优于后听诊组,差异均有统计学意义(P<0.05)。(2)超声心动图指导下心脏听诊组理论考试及实践技能考核总分与分项成绩均高于常规组,差异有统计学意义(P<0.05);先听诊组成绩均高于后听诊组,差异有统计学意义(P<0.05)。(3)先听诊组实践操作技能考核成绩每周均呈上升趋势,差异有统计学意义(F=84.70,P<0.001)。结论超声心动图指导下心脏听诊系统教学法有助于提高医学生的心脏听诊技能,可优化医学生心脏听诊教学。Objective To explore the value of cardiac auscultation under the guidance of echocardiography in optimizing auscultation of cardiac physical examination for medical students.Methods A total of 270 medical students in clinical medicine were divided into two groups:150 students participated in cardiac auscultation teaching under the guidance of echocardiography were divided into pre-auscultation group(75 students)and post-auscultation group(75 students).Conventional auscultation group was 120 medical students.After all completed teaching of the cardiac auscultation,students were assessed through theoretical examinations,practical skills assessment,and a classroom teaching questionnaire.Results(1)The cardiac auscultation group under the guidance of echocardiography was more recognized by students in terms of exerting learning initiative,highlighting key content,and increasing clinical practice ability,highlighting key contents and increasing clinical practice ability,which was significantly different from the conventional clinical auscultation group(P<0.05).Under the guidance of echocardiography,the first auscultation group was better than the later auscultation group in the above aspects,and the difference was statistically significant(P<0.05).(2)Under the guidance of echocardiography,the total scores and sub scores of theoretical examination and practical skill examination in the heart auscultation group were higher than those in the routine group(P<0.05).The results of the first auscultation group were higher than those of the second auscultation group,the difference was statistically significant(P<0.05).(3)The examination results of practical operation skills in the first auscultation group showed an upward trend every week,and the difference was statistically significant(F=84.70,P<0.001).Conclusion The teaching method of cardiac auscultation system under the guidance of echocardiography is helpful for improving the auscultation skills of medical students and can optimize the teaching of cardiac auscultation of
