终末期肾病患者血清镁与左心室质量指数的回顾性分析

摘　　要：目的:探讨终末期肾病(end-stage renal disease,ESRD)未透析患者血清镁与左心室质量指数(left ventricular mass index,LVMI)的相关性。方法:根据研究需要随机选取我院肾脏内科212例未行透析的ESRD患者,测定其血清镁、钙、磷、全段甲状旁腺激素(intact parathyroid hormone,iPTH)等,同时行超声心动图检查,依据Devereux公式计算LVMI并根据左心室肥厚(left ventricular hypertrophy,LVH)的诊断标准将研究对象分为LVH组151例和非左心室肥厚(N-LVH)组61例。结果:单因素分析显示:LVH组在磷、钙磷乘积及收缩压、舒张压、LVMI上均高于N-LVH组(P<0.05),而LVH组血红蛋白、血镁低于N-LVH组(P<0.05);LVMI与钙(r=0.171)、磷(r=0.177)、钙磷乘积(r=0.232)、舒张压(r=0.598)和收缩压(r=0.596)均呈正相关(P<0.05);与血清镁(r=−0.805)、血红蛋白(r=−0.314)呈负相关;多因素分析显示,血清镁及血红蛋白是影响LVMI升高的重要因素。结论:ESRD未透析患者血清镁与LVMI密切相关。Objective:To explore the correlation between serum magnesium and left ventricular mass index in non-dialysis patients with end-stage renal disease(ESRD).Methods:Two hundred and twelve patients with end-stage renal disease(ESRD)who had not received dialysis treatment in our hospital were randomly recruited and their serum magnesium,calcium,phosphorus,intact parathyroid hormone(iPTH)and other clinical indexes were determined.All subjected included underwent echocardiography,and were allocated to left ventricular hyperthopy(LVH)group(n=151)and non-left ventricular hyperthopy(N-LVH)group(n=61)after calculation of the left mass index(LVMI)by Devereux formula.Results:Univariate analysis showed the level of serum phosphorus,calcium-phosphorus product,csystolic blood pressure,diastolic blood pressure,LVMI and iPTH were higher in the LVH group than those in the N-LVH group(P<0.05),while the level of serum magnesium,hemoglobin were lower(P<0.05).LVMI was positively correlated with level of calcium(r=0.171),serum phosphorus(r=0.177),calcium-phosphorus product(r=0.232),diastolic blood pressure(r=0.598)and systolic blood pressure(r=0.596)(P<0.05),whlie was negatively correlated with the level of magnesium(r=−0.805)and hemoglobin(r=−0.314).Multivariate analysis showed that serum magnesium and hemoglobin level were the important risk factors for LVMI.Conclusion:Serum magnesium is closely related to LVMI in non-dialysis patients with ESRD.
