异体脱细胞真皮移植在手部深度烧伤创面治疗中的应用效果

摘　　要：目的研究异体脱细胞真皮移植运用于手部深度烧伤创面治疗的作用及价值。方法前瞻性选取2020年1月至2021年1月亳州市人民医院收治的44例手部深度烧伤创面患者,采用随机数字表法将患者分为试验组与对照组,每组各22例。对照组患者单纯给予实施自体中厚皮片移植,试验组患者则实施异体脱细胞真皮移植结合自体中厚皮片移植治疗。对比两组临床疗效、创面愈合情况、疼痛手功能及创面组织病理学评分以及皮片成活情况及外观平整度。结果试验组的总有效率为95.45%,明显高于对照组(68.18%),差异有统计学意义(P<0.05)。两组患者的创面脱水干燥、创面愈合时间相比,差异均无统计学意义(P>0.05)。治疗后,试验组的手功能评分与创面组织病理学评分为(90.60±7.48)、(1.50±0.46)分,均明显高于对照组[(80.64±8.71)、(1.03±0.40)分],疼痛评分为(2.21±0.24)分,明显低于低于对照组[(3.74±1.36)分],差异均有统计学意义(P<0.05)。试验组的外观平整度以及皮片的成活率为86.36%、90.91%,均高于对照组(54.55%、59.09%),差异有统计学意义(P<0.05)。结论异体脱细胞真皮移植运用于手部深度烧伤创面治疗,疗效较自体中厚皮片移植显著提高,且可有效提高术后手功能恢复水平,减轻术后疼痛程度,降低治疗风险,应用价值较高。Objective To study the effect and value of allogeneic acellular dermis in the treatment of deep hand burn wounds,and to summarize the situation of wound healing,hand function,wound histopathology,etc.,so as to provide reference for clinical related discussion.Methods A total of 44 patients with deep burn wounds on hands admitted to Bozhou People's Hospital from January 2020 to January 20121 were prospectively selected as the observation objects,the patients were divided into the experimental group and the control group,22 cases in each group according to the random number table method.The patients in the control group were simply given autologous medium-thickness skin grafts.The patients in the experimental group were treated with allogeneic acellular dermal transplantation combined with autologous medium-thickness skin grafting.The clinical effect,pain and histopathological grading,hand function and survival rate of the wound healing and skin and appearance were compared between two groups.Results The total effective rate of the experimental group was 95.45%,which was significantly higher than that of the control group(68.18%),and the difference was statistically significant(P<0.05).There was no significant difference in the dehydration and dryness of the wounds and the healing time of deep wounds between the two groups(P>0.05).After treatment,the hand function score and wound histopathology score of the experimental group were(90.60±7.48)points,(1.50±0.46)points,which were significantly higher than those of the control group[(80.64±8.71)points,(1.03±0.40)points],and the pain score was(2.21±0.24)points,which was significantly lower than the control group[(3.74±1.36)points],the difference was statistically significant(P<0.05).The appearance flatness of the test group and the survival rate of the skin were 86.36%,90.91%,which were higher than those of the control group(54.55%,59.09%),and the differences were statistically significant(P<0.05).Conclusion Allogeneic acellular dermal transplantation is used in t
