巴氯芬泵停药反应15例文献分析

摘　　要：目的分析巴氯芬泵停药反应的临床特点,为临床安全用药提供参考。方法检索PubMed、Elsevier Science Direct、Embase、Springer Link、万方、中国知网等数据库,检索时间为从建库至2021年9月30日,收集巴氯芬泵停药反应个案报道,并对患者年龄、性别、适应证、停药原因、症状、停药反应处理及转归等进行统计分析。结果共检索到14篇独立文献报道,病例15例,男性比例为80%。15例患者均因痉挛使用巴氯芬泵,停药原因为巴氯芬泵硬件或软件故障,在停药反应处理方面9例更换泵或导管,7例使用苯二氮䓬类药物或镇静药物缓解肌强直,5例患者口服巴氯芬或鞘内注射巴氯芬替代巴氯芬泵治疗。15例患者在治疗后,痉挛及其他症状和体征均有不同程度的缓解。结论巴氯芬泵停药反应国内少见,但后果严重,应引起临床重视。Objective To analyze the cases of Baclofen pump withdrawal reaction and their clinical characteristics,and provide references for safety of clinical medication.Methods We searched PubMed,Elsevier Science Direct,Embase,Springer Link,Wanfang and CNKI databases from the database establishment time to September 30,2021.Then,we collected case reports of Baclofen pump withdrawal reaction and analyzed the patient's age,gender,indications,causes of withdrawal reaction,symptoms,treatment and outcome of withdrawal reaction.Results A total of 14 independent literature reports were retrieved,including 15 cases,and the proportion of men was 80%.All 15 patients used Baclofen pumps due to spasm.The primary cause of withdrawal reaction was hardware or software failure of the Baclofen pump.In the treatment of withdrawal reaction,9 cases changed pump or catheter;7 cases used Benzodiazepines or sedatives to relieve myotonia,and 5 cases were prescribed oral Baclofen or intrathecal baclofen for Baclofen replacement therapy.After treatment,spasm and other symptoms and signs of 15 patients were relieved to varying degrees.Conclusion Although Baclofen pump withdrawal reaction is rare in China,Its consequences are serious,which should be given more attention.
