摘　　要：目的探讨吸脂联合乳晕切口腺体切除术治疗男性乳房发育(gynecomastia,GYN)的临床效果。方法自2020年1月至2022年3月,中国医学科学院北京协和医院整形美容外科对23例GYN患者采用吸脂联合乳晕切口腺体切除术,评估切口愈合情况,术后并发症发生情况,五级评分量表评估患者满意度。结果本组共23例患者,切口均一期愈合,且无乳头、乳晕坏死等严重并发症发生。术后随访6个月,胸部形态平坦,双侧对称,无明显局部畸形,外观改善明显,患者较满意。结论乳晕下切口腺体切除联合吸脂的术式治疗GYN,手术效果确切,无明显并发症,患者较满意。Objective To investigate the therapeutic effect of liposuction combined with mammary adenectomy through the areola incision in gynecomastia patients.Methods Twenty-three patients diagnosed with idiopathic gynecomastia were recruited in the Department of Plastic Surgery and Reconstructive Surgery,Peking Union Medical College Hospital from January,2020 to March 2022.Preoperative examination and classification were performed.All patients were treated with bilateral mammary gland resection under the areola incision combined with liposuction before and after the adenectomy.During the follow-up period,the wound healing situation and postoperative complications such as hematoma,seroma,infection,nipple areola necrosis,sensory disturbance,deformity and so on were evaluated.Pictures were taken before and after surgical intervention for comparison.Patients were interviewed whether they were satisfied with the aesthetic outcome.Results All gynecomastia breasts has a mixed type of tissue with fat and gland.Incisions all healed in first stage and no serious complications such as nipple areola necrosis occurred.The chest shape were flat and bilaterally symmetrical with no obvious local irregularity.Postoperative appearances improved significantly with which the patients were satisfied.Conclusion Mammary adenectomy through the areola incision combined with liposuction before and after gland resection therapeutically effective for gynecomastia,with no significant complications and high satisfaction.
