摘  要:目的:为了检验太极“云手”对膝骨性关节炎患者锻炼的安全性,给膝骨性关节炎患者提供科学的锻炼方式。方法:使用三维运动分析系统,以26名膝骨性关节炎患者为测试对象,收集和分析了受试者在太极“云手”和“步行”两种步态中下肢关节的运动学和膝关节的动力学参数。结果:两种步态在运动速度和支撑时间上均有显著差异。与“步行”相比,“云手”的踝关节背屈角度、膝关节屈曲角度及髋关节屈曲角度均较大,髋关节的内收角度较小,髋关节外展角度较大,膝关节屈曲力矩和冲量较大,但在内收外力矩和内收冲量上两种活动无显著差异。结论:太极“云手”是一种相对安全、适宜的运动形式,运动中控制适当的膝屈曲角度,可能会增强KOA患者的下肢肌力,提高膝关节的稳定性。Objective: In order to test the safety of Taiji “cloud hand” exercise for patients with knee osteoarthritis and provide scientific exercise methods for patients with knee osteoarthritis. Methods: Using the three - dimensional motion analysis system, taking 26 patients with knee osteoarthritis as test objects, the kinematics of lower limb joints and dynamic parameters of knee joints in Taiji “cloud hand” and “walking” gait were collected and analyzed. Results: There were significant differences in movement speed and support time between the two gait types. Compared with “walking”, the ankle dorsiflexion angle, knee flexion angle and hip flexion angle of “cloud hand” were larger, the hip adduction angle was smaller, the hip abduction angle was larger, and the knee flexion torque and impulse were larger, However, there was no significant difference between the two activities in adduction torque and adduction impulse. Conclusion: Taiji “cloud hand” is a relatively safe and appropriate form of exercise. Controlling an appropriate knee flexion angle during exercise may enhance the lower limb muscle strength and improve the stability of knee joint in patients with KOA.
