基层医疗与三甲医院协作对产前筛查和胎儿畸形临床应用分析

摘　　要：目的探讨基层医疗与三甲医院协作开展产前超声筛查和胎儿畸形诊断的临床应用价值。方法与潍坊市人民医院(三甲医院)进行协作,设立基层医院与三甲医院产前筛查指导站。三甲医院医生每4周到基层医院指导1d/次,由基层医院筛查孕妇被推荐到三甲医院复查,或三甲医院建议孕妇就近到基层医疗协作站进行首次超声筛查。产前筛查适龄孕妇孕周22~26周(产前筛查)后皆做胎儿心脏检查(24~28周)。新冠肺炎疫情(COVID-19)禁止外出接触期除外,共计73例146例次。结果(1)同组孕妇73例次中,两级医院产前筛查结果具有一致性(P=0.0004),而胎儿心脏诊断结果差异无统计学意义(P=0.3173);(2)两级医院超声医师在同组孕妇产前筛查时间、胎儿心脏检查次数上均存在显著差异(P<0.0001)。结论基层医疗与三甲医院协作对基层医院开展产前超声筛查和胎儿畸形超声诊断具有重要的临床应用价值。Objective To explore the clinical application value of cooperation between tertiary to primary care for prenatal screening and fetal malformation.Methods A tertiary academic hospital are selected for cooperation,and guidance station of prenatal screening were set up.Ultrasound experts guide once every 4 weeks.Pregnant women who are screened in primary hospitals are recommended to review in tertiary hospitals.And tertiary hospitals recommend pregnant women to go to primary hospital cooperative stations for their first ultrasound screening.Screening gestational was performed between 22~26 weeks,fetalheart examination was performed between 24~28 weeks.Seventy-three cases and 146 times were examined,except the quarantine period in COVID-19.Results It is consistent for the prenatal screening results of the two hospitals in the same group of 73 cases of pregnant women(P=0.0004),but the results of fetal abnormalities were not statistically significant(P=0.3173).There are significant differences between the two hospital in the screening time and the number of fetal heart tests(P<0.0001).Conclusion Cooperation between tertiary to primary care for prenatal screening and fetal examination has important clinical application value.
