缺血性脑卒中患者急性期抑郁状态及相关因素分析

摘　　要：目的探讨缺血性脑卒中患者急性期出现抑郁状态的情况以及影响因素。方法采用PHQ-9抑郁症筛查量表对2020年11月~2021年3月收治的142例缺血性脑卒中急性期患者进行评估,分为非抑郁状态和抑郁状态,对患者的年龄、性别、职业、婚姻状况、文化程度、缺血性脑卒中严重程度和神经功能评估等因素进行单因素分析,并对其中有统计学意义的因素做Logistic回归分析,探索缺血性脑卒中急性期出现抑郁状态的相关因素。结果142例缺血性脑卒中患者中非抑郁状态66例(46.48%),抑郁状态76例(53.52%)。多元逐步回归分析表明,卒中后急性期抑郁状态与患者年龄、性别无关(P>0.05),而与所患临床症状数量、缺血性卒中病情严重程度和神经功能状况有关(P<0.05)。结论缺血性脑卒中患者急性期出现抑郁状态与缺血性卒中病情严重程度和神经功能缺损程度有关。Objective To investigate the status and influence factors of depression in patients with ischemic stroke at acute stage.Methods The PHQ-9 depression screening scale was used to assess 142 patients admitted from November 2020 to March 2021 in the acute phase of ischemic stroke.Subjects are divided into non-depressed and depressed states.The age,sex,occupation,marital status,education level,severity of ischemic stroke and neurological function assessment of the patients were statistically analyzed by univariate analysis.Logistic regression analysis was done for the statistically significant factors among them to explore the factors associated with the development of depressive states in the acute phase of ischemic stroke.Results Among 142 patients with ischemic stroke,66(46.48%)were in non-depressive state and 76(53.52%)were in depressive state.Multivariate stepwise regression analysis showed that depression in acute stage after stroke was not related to age,gender or severity of patients(P>0.05),but related to clinical symptoms,the severity of the patients and neurological function evaluation after ischemic stroke(P<0.05).Conclusion The presence of depressive states in the acute phase of ischemic stroke patients is related to the severity of ischemic stroke and the extent of neurological deficits.
