摘　　要：目的探讨雌孕激素联合治疗对药物流产后出血患者血管因子及激素的影响。方法回顾性分析80例药物流产出血患者的资料,依据治疗方式分为对照组23例和观察组57例。对照组行常规治疗,观察组行雌孕激素联合治疗。比较治疗后2组阴道出血持续时间、月经复潮时间及子宫内膜厚度;比较2组治疗前后血管内皮生长因子(VEGF)、促血管生成素-1(Ang-1)、促血管生成素-2(Ang-2)和雌二醇(E_(2))、孕激素受体(P)、人绒毛膜促性腺激素(HCG)水平变化。结果治疗后,观察组阴道出血持续时间、月经复潮时间短于对照组,子宫内膜厚度大于对照组,差异有统计学意义(P<0.001)。治疗后,观察组VEGF、Ang-1、Ang-2、HCG水平低于对照组,差异有统计学意义(P<0.001)。治疗后,观察组E_(2)、P水平高于对照组,差异有统计学意义(P<0.001)。结论雌孕激素联合治疗药物流产后出血患者效果较佳,可缩短其阴道出血时间,调节血管生成调节因子和相关激素水平,维持子宫内环境平衡。Objective To investigate the effects of estrogen and progestogen combined therapy on vascular factors and hormones in patients with bleeding after medical abortion.Methods The data of 80 patients with medical abortion bleeding were retrospectively analyzed,and they were divided into control group(n=23)and observation group(n=57)according to treatment methods.The control group received routine treatment,and the observation group received estrogen and progesterone combined therapy.The duration of vaginal bleeding,menstrual revulsion time and endometrial thickness were compared between the two groups after treatment;the levels of vascular endothelial growth factor(VEGF),angiopoietin-1(Ang-1),angiopoietin-2(Ang-2),estradiol(E_(2)),progesterone receptor(P)and human chorionic gonadotropin(HCG)in two groups were compared before and after treatment.Results After treatment,the duration of vaginal bleeding and menstrual revulsion in the observation group was significantly shorter than that in the control group,and the endometrial thickness was significantly greater than that in the control group(P<0.001).After treatment,the levels of VEGF,Ang-1,Ang-2 and HCG in the observation group were significantly lower than those in the control group(P<0.001).After treatment,E_(2) and P levels in the observation group were significantly higher than those in the control group(P<0.001).Conclusion The combination therapy of estrogen and progesterone has a better effect in patients with bleeding after medical abortion,which can shorten the time of vaginal bleeding,regulate the levels of angiogenic regulatory factors and related hormones,and maintain the balance of the uterine environment.
