基于知识图谱分析的国内外整合型医疗卫生服务体系研究

摘　　要：目的了解整合型医疗卫生服务体系研究现状,探讨未来研究趋势。方法运用CiteSpace工具,分别对中国知网(CNKI)数据库和Web of science核心合集数据库中相关文献资料进行科学计量分析,探析整合型医疗卫生服务体系研究领域的科研力量分布、研究热点、热点演变、主题聚类。结果科研力量呈现总体分散、局部集中特点,作者机构间合作程度低;美国发文量最多,中国在国际合作研究方面占有重要地位。国内外在权威期刊的载文量明显不足。国内研究主要围绕整合、医疗资源、一体化、医疗服务等,展开国内整合型医疗卫生服务体系的地方实践与评估,分析及对比部分发达国家的整合模式;国外围绕结果、管理、服务质量进行研究,对整合的理论基础、影响整合的因素和机制设计、特殊群体的具体整合服务方式及效用展开研究。结论针对我国当前研究的不足,认为今后应提升不同机构作者合作力度与研究质量,丰富我国情境下整合型医疗卫生服务体系的内涵研究;完善重点人群整合服务与管理研究;完善整合型医疗服务体系运行机制研究;强化整合医疗服务本土理论基础探索,将理论更好地应用于整合实践。Objective To investigate the research advances in the integrated healthcare systems(IHSs)and explore the future research trends.Methods The CiteSpace tool was used to conduct scientometric analysis of relevant literature data in CNKI and Web of Science Core Collection to explore the distribution of authors,research hotspots,evolution of hotspots,and topic clusters.Results The distribution of authors was featured by overall dispersion and partial concentration,and the degree of cooperation was low among authors from different institutions.Most authors were from the United States,whereas China played an important role in international cooperative research.Only a small proportion of articles were published in top journals in China and abroad.Chinese research mainly focused on the medical resources,integration,and medical services,including practice and evaluation of local IHSs as well as introduction and comparisons with IHSs in some developed countries.Foreign research focused more on the outcomes,management,and service quality,including the theoretical basis of integration,factors affecting integration and mechanism design,and specific integration modes and utilities for special populations.Conclusions In view of the shortcomings of current research on IHSs in China,efforts should be made to enhance the cooperation among authors from different institutions and increase the quality of research,so as to enrich the connotation of IHS research in China.The main research topics may include the integrated services and management in key populations and the operating mechanisms of IHSs.Theoretical research on IHSs will further guide its practice.
