摘 要:目的总结分析某县级医院重点专科建设的实践,探索加强重点专科建设的策略,以不断提升医疗服务能力。方法通过确定专科建设的发展思路,采取发挥政策激励作用、加强人才梯队建设、不断提升服务能力、切实强化质量管理等建设重点专科的主要措施。结果2008年—2020年,医院成功创建了16个市级和11个省级重点专科,位居全省同级医院先进行列。多项医疗服务指标显著提高,用2019年的统计结果与启动重点专科建设的2008年进行比较,门诊量增长了89.27%,达353542人次,住院量增长了169.73%,达37570人次,手术量增长了202.93%,达9733人次;病床周转次数,三、四级手术占比,重点专科较普通专科高危患者占比,住院患者满意度均明显提高,医院县外转诊率显著降低,差异均具有统计学意义(P均<0.05),医疗服务能力明显提升。结论专科建设发展思路的正确引领、倾斜政策的正向激励等主要措施的有效实施,是县级医院成功建设重点专科、提升医疗服务能力的关键;加强跟踪管理、完善建设措施、强化专科联盟是加强重点专科建设的重要途径。Objective To investigate and summarize the construction of key specialties in a county-level hospital and relevant strategies so as to ensure sustained improvement of the medical service capacity.Methods After the vision for the construction of specialties was established,the main measures were developed,including giving play to policy incentives,strengthening talent training,increasing service capabilities,and enhancing quality management.Results From 2008 to 2020,the hospital successfully established 16 municipal and 11 provincial key specialties,ranking high among the county-level hospitals in Hubei Province.Many medical service indicators have been significantly improved.Compared with the statistics in 2018 when the construction of specialties was initiated,the number of outpatient visits increased by 89.27%to 353,542,the number of inpatients increased by 169.73%to 37,570,and the number of surgeries increased by 202.93%to 9,733 in 2019.The hospital bed turnover,proportions of third-and fourth-level surgeries,proportion of high-risk patients in key specialties,and inpatient satisfaction were also significantly improved(P<0.05),along with the decreased referral rate to other the county-level hospital(P<0.05).The healthcare service capacity was significantly improved.Conclusions The effective implementation of main measures such as the correct vision and positive policy-orientated incentives play a key role in the successful construction of key specialties and the improvement of medical service capacity in the county-level hospitals.Optimized tracking system,infrastructure,and alliances with other specialties are significant means in the construction of key specialties.
