超声联合结核菌素皮试、血清腺苷脱氨酶诊断附睾结核的价值

摘　　要：目的探究高分辨率超声(HR-US)联合结核菌素皮试(TST)、血清腺苷脱氨酶(ADA)对附睾结核的诊断价值。方法选取2017年3月-2020年2月本院收治的53例附睾结核患者为附睾结核组,另选取53例男性健康志愿者为对照组。以组织病理学诊断为金标准,分析HR-US、TST、血清ADA及三者联合对附睾结核的诊断价值。结果附睾结核组患者血清ADA水平(28.35±11.34)U/L高于对照组(13.52±5.41)U/L(P<0.05);HR-US、TST、血清ADA及三者联合诊断附睾结核的灵敏度分别为84.91%、79.25%、77.36%、98.11%,特异度分别为92.45%、88.68%、90.57%、86.79%,准确度分别为88.68%、83.96%、83.96%、92.45%。结论HR-US联合TST、血清ADA对附睾结核有较高的诊断价值,有助于临床高效筛查附睾结核患者。Objective To explore the diagnostic value of high-resolution ultrasonography(HR-US)combined with tuberculin skin test(TST)and serum adenosine deaminase(ADA)for epididymal tuberculosis.Methods A total of 53 patients with epididymal tuberculosis admitted to our hospital during Mar.2017 and Feb.2020 were selected as the epididymal tuberculosis group,and 53 healthy male volunteers were enrolled as the control group.The diagnostic value of HR-US,TST,serum ADA,and the combination of the three in the diagnosis of epididymal tuberculosis was analyzed with histopathological diagnosis as the gold standard.Results The level of serum ADA was higher in the epididymal tuberculosis group[(28.35±11.34)U/L]than in the control group[(13.52±5.41)U/L](P<0.05).The sensitivity of HR-US,TST,serum ADA and their combination in the diagnosis of epididymal tuberculosis were 84.91%,79.25%,77.36%and 98.11%,respectively;the specificity were 92.45%,88.68%,90.57%and 86.79%,respectively;the accuracy were 88.68%,83.96%,83.96%and 92.45%,respectively.Conclusion HR-US combined with TST and serum ADA has a high diagnostic value for epididymal tuberculosis,which is helpful for the efficient screening of patients with epididymal tuberculosis.
