弹性髓内针固定联合经皮穿刺人工骨注射治疗儿童肱骨单纯性骨囊肿:31例中期随访结果

摘　　要：目的探讨弹性髓内针固定联合人工骨注射治疗儿童肱骨单纯性骨囊肿的疗效。方法2015年6月~2019年6月,对31例儿童肱骨单纯性骨囊肿,在C臂机透视下,在肱骨内、外上髁分别做1 cm切口,由肱骨远端向近端置入2枚弹性髓内针固定,经皮穿刺注射人工骨填充病灶。结果手术时间60~95 min,(68.0±10.5)min;术中出血量20~50 ml,(34.7±8.7)ml。31例随访36~65个月,(49.6±10.8)月。按改良Neer标准,Ⅰ级(愈合)22例,Ⅱ级(部分愈合)8例,Ⅲ级(持续存在)1例,无Ⅳ级(复发),治疗有效率96.8%(30/31)。无感染、病理性骨折。结论采用弹性髓内针固定联合经皮穿刺人工骨注射治疗儿童肱骨单纯性骨囊肿疗效满意。Objective To investigate the efficacy of elastic intramedullary nailing fixation combined with artificial bone injection in minimally invasive treatment of simple bone cyst of humerus in children.Methods From June 2015 to June 2019,31 children with simple bone cyst of humerus were treated.Under the C-arm X-ray fluoroscopy,a 1 cm incision was made in the inner and outer epicondyles of the humerus,and two elastic intramedullary needles were implanted from the distal end of the humerus to the proximal end for fixation.Percutaneous puncture of artificial bone injection was conducted to fill the lesion.Results The operation time was 60-95 min(mean,68.0±10.5 min);the intraoperative blood loss was 20-50 ml(mean,34.7±8.7 ml).All the 31 cases were followed up for 36-65 months(mean,49.6±10.8 months).According to the modified Neer classification of healing,22 cases healed in gradeⅠ(healed),8 cases reached gradeⅡhealing(healing with defects),1 case was gradeⅢ(persistent cyst),and 0 case gradeⅣ(recurrent cyst),the effective rate being 96.8%(30/31).No infection or pathological fracture occurred.Conclusion Elastic intramedullary nailing fixation combined with percutaneous artificial bone injection has a satisfactory result in the treatment of simple bone cyst of humerus in children.
