摘　　要：目的探讨循证护理(EBN)方法应用于儿童川崎病(Kawasaki disease,KD)症状护理、特殊用药护理以及心理护理的临床效果。方法选取苏州大学附属儿童医院2019年10月至2020年3月诊断为KD的患儿168例,按入院时间分组,前3个月住院的病例82例,采用传统护理方法,作为对照组;后3个月住院的病例86例,采用EBN方法护理,作为观察组。比较两组临床症状消退时间、用药不良反应、家属满意度、依从性、焦虑程度等指标。结果观察组与对照组患儿皮疹消退时间、球结膜充血消退时间差异无统计学意义(均P>0.05),观察组口唇干裂愈合时间较对照组提前(P=0.0001)。两组用药不良反应发生率差异无统计学意义(均P>0.05)。治疗后观察组家属焦虑自评量表评分较对照组显著降低,差异有统计学意义(P<0.001)。观察组护理满意度、服药依从性、随访依从性均高于对照组,差异均有统计学意义(均P<0.05)。结论EBN有助于早期改善KD病人临床症状,降低用药不良反应发生率,减轻家属焦虑程度,提高临床护理满意度,从而提高服药依从性及随访依从性,值得推广。Objective To explore the clinical effect of evidence-based nursing(EBN)in symptom nursing,special medication nursing and psychological nursing of children with kawasaki disease(KD).Methods A total of 168 children diagnosed with KD from October 2019 to March 2020 were selected.According to the time of admission,82 cases in the first 3 months of hospitalization,using traditional nursing methods,as the control group.In the latter 3 months,86 patients were hospitalized and treated with EBN method as the observation group.The clinical symptom regression time,adverse drug reactions,family satisfaction,compliance,and anxiety were compared.Results There were no statistically significant differences in the resolving time of skin rash and conjunctival hyperemia between the observation group and the control group(P>0.05).The healing time of the observation group was earlier than that of the control group(P=0.0001).There was no significant difference in adverse drug reaction rates between the two groups(P>0.05).After treatment,the scores of family anxiety self-rating scale in the observation group were significantly lower than those in the control group(P<0.001).The nursing satisfaction,medication compliance and follow-up compliance of observation group were higher than those of control group,the differences were statistically significant(P<0.05).Conclusions EBN is helpful to improve the clinical symptoms of patients with KD,reduce the incidence of adverse drug reactions,reduce the anxiety of family members,and improve the clinical nursing satisfaction,so as to improve the medication compliance and follow-up compliance.It is worth promoting.
