乳牙活髓切断术及间接盖髓术治疗乳磨牙深龋近髓的临床效果观察

摘　　要：目的观察乳牙活髓切断术及间接盖髓术对乳磨牙深龋近髓的临床疗效。方法选择2018年5月—2020年5月收治的乳磨牙深龋近髓患儿140例,按治疗方法分为观察组70例(88颗)与对照组70例(91颗)。对照组给予间接盖髓术治疗,观察组给予活髓切断术治疗。比较两组治疗后3、6、9、12个月患牙存活率、成功率及牙本质桥形成率,并于治疗后12个月比较两组并发症发生情况。结果治疗后9、12个月,观察组患牙存活率和成功率高于对照组(P<0.05)。治疗后6、9、12个月,观察组牙本质桥形成率高于对照组(P<0.05)。两组治疗后12个月并发症总发生率比较差异无统计学意义(P>0.05)。结论活髓切断术用于乳牙深龋近髓的治疗效果优于间接盖髓术,可促进牙本质桥形成,且安全性较高。Objective To observe clinical efficacies of vital pulpotomy and indirect pulp capping in treatment of deciduous molars with deep caries and proximal pulp.Methods A total of 140 children with deep caries proximal pulp of deciduous molars who had been treated between May 2018 and May 2020 were selected,and they were divided into observation group(n=70,88 teeth)and control group(n=70,91 teeth)according to therapeutic methods.Control group was treated with indirect pulp capping,while observation group was treated with vital pulpotomy.Survival rates,success rates and dentin bridge formation rates after treatment for 3,6,9,and 12 months were compared between two groups,and incidence rate of complications after treatment for 12 months was compared between two groups.Results After treatment for 9 and 12 months,survival rates and success rates of the affected teeth in observation group were higher than those in control group(P<0.05).After treatment for 6,9,and 12 months,dentin bridge formation rates in observation group were higher than those in control group(P<0.05).There was no significant difference in the total incidence rate of complications between two groups after treatment for 12 months(P>0.05).Conclusion The efficacy of vital pulpotomy in treatment of deciduous molars with deep caries and proximal pulp is better than that by indirect pulp capping,which may promote the formation of dentin bridge with good safety.
