穴位贴敷法联合托烷司琼对全身麻醉术后恶心呕吐的影响

摘　　要：目的:探讨穴位贴敷法联合托烷司琼对全身麻醉术后恶心呕吐(PONV)的影响。方法:选择2019年7月-2021年9月符合纳入标准的桂林医学院第二附属医院行全身麻醉的妇科腹腔镜的手术患者360例,采取临床随机对照试验,将其随机分为A、B、C、D四组(n=90),A组手术结束前30 min静脉推注托烷司琼5mg+术后30 min进行穴位中药贴敷持续24 h;B组术后30 min进行穴位中药贴敷持续24 h;C组手术结束前30 min静脉推注托烷司琼5 mg;D组术后30 min进行穴位生理盐水贴敷持续24 h。对比四组PONV发生情况、术后恢复质量。结果:四组PONV发生率比较,差异有统计学意义(P<0.05),A组最低,为7.78%。四组术后T_(1)、T_(2)时刻VAS评分对比,差异无统计学意义(P>0.05),A组患者术后T_(3)、T_(4)时刻VAS评分分别为(1.76±0.53)、(1.32±0.87)分,均明显低于B组、C组、D组(F=15.951、13.834,P<0.05);四组术后第1天QOR-15量表评分对比,差异无统计学意义(P>0.05),A组患者术后第2、3天QOR-15量表评分分别为(98.20±15.64)、(106.52±13.24)分,均明显高于B组、C组、D组(F=19.719、29.541,P<0.05)。结论:对于全身麻醉的妇科腹腔镜手术后患者,采取穴位贴敷法联合托烷司琼能最大限度地降低PONV的发生率,减轻恶心程度,提高患者恢复质量,有益于临床推广。Objective:To investigate the effect of acupoint application combined with Tropisetron on postoperative nausea and vomiting(PONV)after general anesthesia.Method:A total of 360 cases of gynecological laparoscopic surgery patients who met the inclusion criteria and underwent general anesthesia from July 2019 to September 2021 in the Second Affiliated Hospital of Guilin Medical College were selected.Randomized clinical trials were conducted and they were randomly divided into group A,group B,group C,and group D(n=90).Group A was taken intravenous boluses of Tropisetron 5 mg at 30 minutes before the end of the operation and acupoint application of traditional Chinese medicine at 30 minutes after the operation for 24 hours.Group B was taken acupoint application of traditional Chinese medicine at 30 minutes after the operation for 24 hours.Group C was intravenously injected with Tropisetron 5 mg at 30 minutes before the end of operation.Group D received acupoint saline application at 30 min after surgery for 24 hours.The incidence of PONV and postoperative recovery quality were compared between the four groups.Result:The incidence of PONV in four groups were significant(P<0.05),the group A(7.78%)was lowest.VAS scores of the four groups at T_(1) and T_(2) showed no significant difference(P>0.05).In group A,VAS scores at T_(3) and T_(4) were(1.76±0.53)and(1.32±0.87)points respectively,which were significantly lower than those of group B,group C,and group D(F=15.951,13.834,P<0.05).There was no significant difference in QOR-15 scale scores between the four groups on the first day after surgery(P>0.05),the QOR-15 scale scores of the patients in group A were(98.20±15.64)and (106.52±13.24)points on the 2nd and 3rd day after operation,which were significantly higher than those in group B,group C,and group D(F=19.719,29.541,P<0.05).Conclusion:For gynecological patients under general anesthesia after laparoscopic surgery,acupoint application combined with Trotansetron can reduce the incidence of PONV to the greatest extent,re
