摘　　要：目的:探究Devine术中阴茎根部与背侧皮下固定治疗真性隐匿性阴茎(concealed penis,CP)患儿的应用效果。方法:回顾性收集福州儿童医院98例真性CP患儿(2019年1月-2021年3月),按治疗方案不同分成A组(n=49)、B组(n=49)。两组均行Devine术治疗,B组接受阴茎根部、腹侧皮下固定,A组接受阴茎根部与背侧皮下固定。对比两组围手术期指标,术前、术后1、3个月非勃起阴茎长度,术后3、7 d视觉模拟评分法(VAS)评分,术前、术后24、48 h创伤定量指标水平,术后并发症发生率。结果:两组术中失血量对比,差异无统计学意义(P>0.05);A组手术时长、水肿时长均短于B组(P<0.05);术后1、3个月两组非勃起阴茎长度长于术前,且A组长于B组(P<0.05);A组术后3、7 d的VAS评分低于B组(P<0.05);术后24、48 h两组血清CRP、IL-6水平均高于术前,且A组低于B组(P<0.05);两组并发症总发生率对比,差异无统计学意义(P>0.05)。结论:Devine术中阴茎根部与背侧皮下固定治疗真性CP患儿可进一步缩短手术时长、水肿时长,减轻术后疼痛感,延长非勃起阴茎长度,减轻机体创伤,安全可行。Objective:To investigate the application effect of Devine’s technique in the treatment of children with true concealed penis(CP)with penile root and dorsal subcutaneous fixation.Method:A total of 98 children with true CP in Fuzhou Children’s Hospital from January 2019 to March 2021 were retrospectively collected and divided into group A(n=49)and group B(n=49)according to different treatment plans.Devine’s technique was performed in both groups.Group B received penile root and ventral subcutaneous fixation,and group A received penile root and dorsal subcutaneous fixation.Perioperative indicators,non-erect penis length before surgery,1 month and 3 months after surgery,visual analogue scale(VAS)scores at 3 and 7 days after surgery,trauma quantitative indicators at before surgery,24 and 48 hours after surgery,and incidence of postoperative complications were compared between the two groups.Result:There was no significant difference in intraoperative blood loss between the two groups(P>0.05).The duration of operation and edema in group A were shorter than those in group B(P<0.05).At 1 and 3 months postoperatively,the length of non-erect penis in two groups was longer than those before operation,and group A was longer than that in group B(P<0.05).The VAS scores of group A were lower than those of group B at 3 and 7 days after operation(P<0.05).The levels of CRP and IL-6 in both groups at 24 h and 48 h after operation were higher than those before operation,and group A were lower than those of group B(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion:Devine’s technique intraoperative penile root and dorsal subcutaneous fixation for the treatment of children with true CP can further shorten the duration of surgery and edema,relieve postoperative pain,prolong the length of non-erect penis,and relieve body trauma,which is safe and feasible.
