EBV抗体联合DNA检测诊断小儿呼吸道EB病毒感染的应用价值

摘　　要：目的:研究EB病毒(Epstein-Barr virus,EBV)抗体与DNA联合检测对儿童呼吸道EBV感染性疾病诊断中的应用价值。方法:选取2019年12月-2021年11月临沂市第三人民医院收治的呼吸道感染患儿360例纳入研究,均于入院时进行EBV抗体和EBV-DNA检测,根据EB病毒诊断金标准分为EB组(174例)和非EB组(186例)。对比EBV抗体检测、EBV-DNA检测及其联合检测的诊断效能。结果:EBV抗体联合EBV-DNA检测准确度为94.72%,高于抗体检测的80.28%和EBV-DNA检测的80.56%,特异度、敏感度均高于单一检测;EBV-IgA的诊断敏感度和准确度低于EBV-IgG和EBV-IgM,差异有统计学意义(P<0.05)。结论:EBV抗体检测和EBV-DNA检测对于鉴别儿童呼吸道EB病毒感染有一定作用,但二者联合检测其诊断效能更好,具有更高临床价值,值得广泛应用。Objective:To investigate the value of combined Epstein-Barr virus(EBV)antibody and DNA testing in the diagnosis of pediatric respiratory tract EBV infection.Method:A total of 360 children with respiratory tract infection admitted to the Linyi Third People’s Hospital from December 2019 to November 2021 were selected for inclusion in the study,all of whom were tested for EBV antibody and EBV-DNA on admission,and were divided into EB group(174 cases)and non-EB group(186 cases)based on gold standard for EB virus diagnosis.The diagnostic efficacy of EBV antibody detection,EBV-DNA detection and their combined detection was compared.Result:The accuracy of EBV antibody combined with EBV-DNA test was 94.72%,which was higher than 80.28%of antibody test and 80.56%of EBV-DNA test,the specificity and sensitivity were higher than those of single test;the diagnostic sensitivity and accuracy of EBV-IgA were lower than those of EBV-IgG and EBV-IgM,and the differences were statistically significant(P<0.05).Conclusion:EBV antibody detection and EBV-DNA detection play a certain role in differentiating respiratory tract EBV infection in children,but their combined detection has better diagnostic efficacy and higher clinical value,which is worthy of widespread application.
