术中优化护理在超声引导胸椎旁神经阻滞下胸腔镜肺癌根治术中的应用效果

摘　　要：目的:分析术中优化护理在超声引导胸椎旁神经阻滞下胸腔镜肺癌根治术中的应用效果。方法:收集2019年12月-2021年3月福建医科大学附属协和医院收治的行超声引导胸椎旁神经阻滞下胸腔镜肺癌根治术的102例患者作为研究对象,随机分为对照组和试验组,各51例。对照组予以术中常规护理,试验组予以术中优化护理。比较两组的围手术期指标、应激反应及不良反应发生情况。结果:试验组的手术时间、住院时间、麻醉苏醒时间、胸管拔除时间及尿管拔除时间均优于对照组,同时术中出血量也少于对照组,差异有统计学意义(P<0.05);术毕时,试验组的乳酸水平、心率值、收缩压及舒张压均低于对照组,差异有统计学意义(P<0.05);试验组不良反应的发生率为3.92%,低于对照组的15.69%,差异有统计学意义(P<0.05)。结论:术中优化护理在超声引导胸椎旁神经阻滞下胸腔镜肺癌根治术中的应用效果显著,有助于改善患者围手术期指标,缓解应激反应,并且预防不良反应发生。Objective:To analyze the application effect of optimized intraoperative nursing in thoracoscopic radical resection of lung cancer under ultrasound-guided thoracic paraspinal nerve block.Method:A total of 102 patients who were underwent thoracoscopic radical resection of lung cancer under ultrasound-guided thoracic paraspinal nerve block from December 2019 to March 2021 in Union Hospital of Fujian Medical University were selected as the research objects,they were randomly divided into the control group and the experimental group,each with 51 cases.The control group was received routine care during the operation,the experimental group was given optimized intraoperative nursing during the operation.The perioperative indicators,stress reaction and occurrence of adverse reactions were compared between the two groups.Result:The operation time,hospitalization time,awakening time from anesthesia,chest tube removal time and urinary tube removal time of the experimental group were better than those of the control group,and the amount of intraoperative blood loss was also lower than that of the control group,the differences were statistically significant(P<0.05);at the end of the operation,the lactic acid level,heart rate value,systolic blood pressure and diastolic blood pressure in the experimental group were lower than the control group,the differences were statistically significant(P<0.05);the incidence of adverse reaction in the experimental group was 3.92%,which was lower than 15.69%in the control group,and the difference was statistically significant(P<0.05).Conclusion:Optimized intraoperative nursing has a significant effect in the application of thoracoscopic radical resection of lung cancer under ultrasound-guided thoracic paravertebral nerve block,which helps to improve patients’perioperative indicators,relieve stress response,and prevent adverse reactions.
