摘　　要：目的:分析鼻内镜下低温等离子射频消融术治疗鼾症伴过敏性鼻炎(AR)患儿的效果。方法:选取2018年1月-2021年8月北京市顺义区妇幼保健院收治的147例鼾症伴AR患儿作为研究对象。按随机数字表法将其分为对照组(73例)和观察组(74例)。观察组给予鼻内镜下低温等离子射频消融术治疗,对照组给予扁桃体联合腺样体切除术治疗。比较两组围手术期指标及临床疗效,术后24 h应激指标,术前、术后3个月时通气功能。结果:观察组手术时间及住院时间均短于对照组,术中出血量少于对照组,并发症发生率及术后1、3、5 d儿童特殊疼痛行为反应评分量表(FLACC)评分均低于对照组(P<0.05)。观察组治疗总有效率高于对照组(P<0.05)。术后24 h,观察组超氧化物歧化酶(SOD)、谷胱甘肽(GSH)水平均高于对照组,而丙二醛(MDA)、硝基酪氨酸(3-NT)水平均低于对照组(P<0.05)。术后3个月,观察组最长呼吸暂停时间(LAT)、氧减指数(ODI)、呼吸紊乱指数(AHI)均低于对照组,最低夜间血氧饱和度(LSaO_(2))高于对照组(P<0.05)。结论:小儿鼾症伴AR的手术治疗中,鼻内镜下低温等离子射频消融术具有视野清晰、创伤小、疼痛轻等优势,可减少术后并发症,改善机体应激反应,提高通气功能,具有重要的研究价值。Objective:To analyze the effect of low temperature plasma radiofrequency ablation under nasal endoscope in the treatment of children with snoring and allergic rhinitis(AR).Method:A total of 147 children with snoring and AR treated in Shunyi District Maternal and Child Health Hospital of Beijing from January 2018 to August 2021 were selected as the research objects.They were divided into control group(73 cases)and observation group(74 cases)according to the random number table method.The observation group was given the low temperature plasma radiofrequency ablation under nasal endoscope treatment,and the control group was given the tonsillectomy combined with adenoidectomy treatment.The perioperative indexes and clinical efficacy,stress indexes 24 hours after surgery and ventilation function before and 3 months after surgery were compared between the two groups.Result:The surgery time and hospital stay in the observation group were shorter than those in the control group,the amount of intraoperative bleeding was less than that in the control group,the incidence of complications,and the scores of children’s special pain behavior response scale(FLACC)at 1,3 and 5 days after surgery were lower than those in the control group(P<0.05).The total effective rate of the observation group was higher than that of the control group(P<0.05).24 hours after surgery,the levels of superoxide dismutase(SOD)and glutathione(GSH)in the observation group were higher than those in the control group,while the levels of malondialdehyde(MDA)and nitrotyrosine(3-NT)were lower than those in the control group(P<0.05).Three months after surgery,the longest apnea time(LAT),oxygen desaturation index(ODI)and respiratory disorder index(AHI)in the observation group were lower than those in the control group,and the lowest nocturnal oxygen saturation(LSaO_(2))was higher than that in the control group(P<0.05).Conclusion:In the surgical treatment of snoring with AR in children,low temperature plasma radiofrequency ablation under nasal endoscope has t
