经椎间孔选择性神经根封闭在腰椎间盘突出症治疗中的临床观察

摘　　要：目的:探讨经椎间孔选择性神经根封闭术在腰椎间盘突出症治疗中的临床应用。方法:收集中国人民解放军联勤保障部队第九八五医院2017年5月-2020年5月行经椎间孔神经根封闭治疗的腰椎间盘突出症资料,利用疼痛视觉模拟评分(VAS)、Oswetry功能障碍指数(ODI)及MacNab评分量表评估其疗效。结果:术后次日、术后3个月及末次随访(术后12个月)时VAS评分均值、ODI评分均值均明显低于术前,差异均有统计学意义(P<0.05);术后次日、术后3个月及末次随访MacNab评估手术优良率分别为70.59%、76.47%、64.71%,所有病例无并发症发生。结论:经椎间孔选择性神经根封闭术治疗腰椎间盘突出症安全有效,值得在临床中广泛推广。Objective:To explore the clinical application of transforaminal selective nerve root block in the treatment of lumbar disc herniation.Method:The data of patients with lumbar disc herniation who underwent transforaminal nerve root block treatment from May 2017 to May 2020 in the 985th Hospital of the PLA Joint Logistics Supports Force were collected,and the efficacy was evaluated by visual analogue score(VAS),Oswetry dysfunction index(ODI)and MacNab scale.Result:The average value of VAS scores and the average value of ODI scores at the next day after operation,3 months after operation and the last follow-up(12 months post-operation)were significantly lower than preoperative,and the differences were statistically significant(P<0.05).The excellent and good rates of surgery assessed by MacNab at 3 months postoperatively and at the last follow-up were 70.59%,76.47%and 64.71%,respectively.No complications occurred in all cases.Conclusion:Transforaminal selective nerve root block is safe and effective for the treatment of lumbar disc herniation,and it is worthy of being widely promoted in clinical practice.
