颈三针联合固本舒筋汤治疗神经根型颈椎病的临床研究

摘　　要：目的:探讨颈三针联合固本舒筋汤在神经根型颈椎病患者中的治疗效果。方法:选取2019年4月-2021年4月龙岩市第二医院接收的87例神经根型颈椎病患者为研究对象,采用随机数字表法分为观察组(43例)和对照组(44例);两组均接受常规治疗措施,在此基础上,对照组采用颈三针治疗,观察组采用颈三针联合固本舒筋汤治疗,两组均治疗4周,对所有患者进行为期6个月的跟踪随访。比较两组治疗前、治疗4周时的颈椎功能[采用颈椎功能障碍指数量表(NDI)评估]、颈椎活动度与疼痛程度[采用简化McGill疼痛问卷(SF-MPQ)评估],并比较两组随访期间神经根型颈椎病复发情况。结果:治疗4周时,观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05);治疗4周时,两组NDI评分均较治疗前降低,且观察组低于对照组,差异有统计学意义(P<0.05);治疗4周时,两组左旋转、右旋转、左侧屈、右侧屈活动度均较治疗前升高,且观察组高于对照组,差异有统计学意义(P<0.05);治疗4周时,两组SF-MPQ量表各部分评分均较治疗前降低,且观察组低于对照组,差异有统计学意义(P<0.05);随访期间,观察组有2例复发,复发率为4.88%(2/41),对照组有8例复发,复发率为22.86%(8/35),两组复发率比较,差异有统计学意义(χ=3.884,P=0.049)。结论:神经根型颈椎病患者采用颈三针联合固本舒筋汤治疗,可改善颈椎功能,提高颈椎活动度,减轻疼痛程度,降低神经根型颈椎病复发率。Objective:To explore the therapeutic effect of cervical three-needle combined with Guben Shujin Decoction in patients with nerve root type cervical spondylopathy.Method:A total of 87 patients with nerve root type cervical spondylopathy received from April 2019 to April 2021 in Second Hospital of Longyan City were selected as the research objects,and were divided into observation group(43 cases)and control group(44 cases)by random number table method;two groups of patients received conventional treatment measures.On this basis,the control group was treated with cervical three-needle,and the observation group was treated with cervical three-needle combined with Guben Shujin Decoction.Both groups were treated for 4 weeks.The cervical spine function[assessed by the neck dysfunction index(NDI)],cervical spine range of motion and pain[assessed by the short-form of McGill question-naire(SF-MPQ)]before and 4 weeks of treatment were compared between the two groups.Result:After 4 weeks of treatment,the total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After 4 weeks of treatment,the NDI scores in two groups were decreased,the observation group was lower than the control group,the difference was statistically significant(P<0.05).After 4 weeks of treatment,the left rotation,right rotation,left flexion,and right flexion range of motion in the two groups were all higher than those before treatment,and the observation group was higher than the control group,and the differences were as follows:Statistical significance(P<0.05);after 4 weeks of treatment,the scores of each part of the SF-MPQ scale in the two groups were lower than those before treatment,and the observation group was lower than the control group,the differences were statistically significant(P<0.05).During the follow-up period,there were 2 cases of recurrence in the observation group,with a recurrence rate of 4.88%(2/41),and 8 cases of recurrence in the control group,with
