从五脏气化论阳痿辨治思路

摘　　要：五脏气化是气血津液等基本生命物质生化、排泄的基础,人体组织器官正常运行赖于五脏气化对人体的调控。五脏气化对阴茎正常勃起有至关重要的作用,五脏气化功能失调皆可致阳痿。具体而言,心气不足、心神失养、心阳不振可致阳痿,治宜益气养心,安养心神,振奋心阳;肺气不足、气血不畅、肺失宣肃可致阳痿,治宜补肺益气,调畅气血,清热利湿;脾气亏虚、血失统摄可致阳痿,治宜健脾益气,濡养宗筋;肝气郁滞、肝气虚而不升、肝血不足可致阳痿,治宜疏肝理气,养血助肝;肾气亏虚、阴阳失和亦可致阳痿,治宜补肾益气,调和阴阳。通过阐述五脏气化理论依据,从五脏气化功能失调论治阳痿,辨证与辨病结合,灵活遣方用药,为中医药治疗阳痿提供理论依据。Qi transformation of five zang-organs is the basis for the generation,transformation and excretion of basic life substances such as qi,blood and body fluids.The normal operation of human tissues and organs depends on the regulation of qi transformation of five zang-organs on the human body.Qi transformation of five zang-organs plays a vital role in the normal erection of the penis,and any dysfunction of qi transformation of five zang-organs can lead to impotence.Specifically,insufficiency of heart qi,malnutrition of heart spirit,and lack of heart yang can cause impotence,which can be treated by replenishing qi and nourishing heart,calming heart spirit,and invigorating heart yang.Deficiency of lung qi,unsmooth circulation of qi and blood,and failure of lung in ascent purification and descent can lead to impotence,which can be treated by tonifying lung and replenishing qi,regulating qi and blood,clearing heat and draining dampness.Deficiency of spleen qi failing to control blood can cause impotence,which can be treated by soothing liver and regulating qi,nourishing blood and assisting liver.Deficiency of kidney qi and disharmony of yin and yang can also cause impotence,which can be treated by tonifying lung and replenishing qi,harmonizing yin and yang.This paper expounds the theoretical basis of qi transformation of five zang-organs,analyzes the treatment of impotence from the dysfunction of qi transformation of five zang-organs,combines syndrome differentiation and disease differentiation,and flexibly summarizes the medication and formulation to provide a theoretical basis for the treatment of impotence with traditional Chinese medicine.
