古代医家针灸治疗脱肛数据挖掘分析

摘　　要：以《中华医典》为主要检索源,收录符合纳入标准的清代及清代以前医家针灸治疗脱肛的相关文献,对治疗经验进行挖掘与总结。脱肛具有明显的肛周局部症状,病因病机主要责之脏腑功能失调。该病主要分为虚证和实证,虚证可由机体虚弱、中气不足、气血亏虚等导致,实证可由胃肠积热、湿热壅滞等导致,临床上亦有虚实错杂证。通过数据挖掘总结出针灸治疗脱肛的处方规律:以单穴处方为主,重视灸法的应用;经脉主要选取督脉、任脉、足太阳膀胱经及足少阴肾经;选用腧穴主要分布在背腰部、头面部和胸腹部;重视交会穴、络穴、八脉交会穴和背俞穴的应用;通过局部选穴、上下选穴和循经选穴,使局部治疗和脏腑功能的整体调节相结合,可有效提高疗效。Taking Chinese Medical Classics as the main search source,we collected relevant literature on acupuncture and moxibustion in the treatment of proctoptosis by physicians in and before the Qing Dynasty that met the inclusion criteria,and excavated and summarized the treatment experience.The proctoptosis has obvious local symptoms around the anus.The etiology and pathogenesis are mainly due to the dysfunction of zang-fu organs.The disease is mainly divided into deficiency syndrome and excess syndrome.Deficiency syndrome can be caused by weakness of the body,insufficiency of middle qi and deficiency of qi and blood.Excess syndrome can be caused by gastrointestinal heat accumulation and stagnation of dampness and heat.Clinically,there is also deficiency and excess in complexity syndrome.Through data mining,the formula regularity of acupuncture and moxibustion in the treatment of proctoptosis is summarized.Focusing on single point formula and paying attention to the application of moxibustion.The main meridians are governor vessel,conception vessel,bladder meridian of foot Taiyang and kidney meridian of foot Shaoyin.The selected acupoints are mainly distributed in the back and waist,head and face,chest and abdomen.Paying attention to the application of crossing point,collateral point,confluence points of eight extraordinary meridians and back transport point.Through local acupoint selection,superior-inferior point combination and acupoint selection along the meridian,the combination of local treatment and the overall regulation of visceral function can effectively improve the efficacy.
