摘 要:目的:探讨3D打印个体化导板治疗无神经损伤胸腰椎骨折的临床效果。方法:选94例患者根据手术方案分为两组,经皮组用经皮椎弓根螺钉固定术,联合组用3D打印个体化导板辅助治疗。比较两组临床指标。结果:联合组透视次数7.18±1.39、出血量63.27±9.85、住院时间7.26±1.93、手术时间71.52±11.37对比对照组(P<0.05);联合组VAS评分1.61±0.32低于对照组,联合组后凸Cobb角6.09±1.57、椎体前缘高度比94.26±10.17对比对照组(P<0.05);联合组髋关节功能Harris评分78.62±7.35、椎体复位率89.63±3.52、椎弓根置入准确率92.14±3.58高于经皮组(P<0.05)。结论:3D打印个体化导板辅助经皮椎弓根螺钉固定可提升骨折复位效果。Objective:To investigate the clinical effect of 3D printed individualized guide plate in the treatment of thoracolumbar fractures without nerve injury.Methods:94 patients were divided into two groups according to the operation scheme.The percutaneous group was treated with percutaneous pedicle screw fixation,and the combined group was treated with 3D printed individualized guide plate.The clinical indexes of the two groups were compared.Results:in the combined group,the fluoroscopy times were 7.18±1.39,the amount of bleeding was 63.27±9.85,the hospital stay was 7.26±1.93 and the operation time was 71.52±11.37 compared with the control group(P<0.05);The VAS score of the combined group was 1.61±0.32 lower than that of the control group,the kyphosis Cobb angle of the combined group was 6.09±1.57,and the height ratio of the anterior edge of the vertebral body was 94.26±10.17 compared with the control group(P<0.05);The Harris score of hip function in the combined group was 78.62±7.35,the reduction rate of vertebral body was 89.63±3.52,and the accuracy rate of pedicle implantation was 92.14±3.58,which was higher than that in the percutaneous group(P<0.05).Conclusion:3D printed individualized guide plate assisted percutaneous pedicle screw fixation can improve the effect of fracture reduction.
