补肾活血汤治疗膝骨关节炎临床疗效及安全性的Meta分析

摘　　要：目的:系统评价补肾活血汤治疗膝骨关节炎(KOA)的临床疗效及安全性。方法:基于中、英文数据库手动检索相关期刊,检索时间自建库至2021年7月。由两名研究员筛选纳入文献、提取所需资料并评价纳入文献质量后,采用RevMan 5.3软件进行Meta分析。结果:共纳入10篇文献,共10项研究,1181例患者。Meta分析显示,补肾活血汤所在研究组治疗KOA在提高总有效率[OR=3.80,95%CI(2.53,5.72),Z=6.42,P<0.00001]、降低视觉模拟评分法(VAS)[MD=-1.60,95%CI(-2.57,-0.63),P=0.001]、抑制白细胞介素-1(IL-1)[SMD=-1.34,95%CI(-2.21,-0.47),P=0.003]、减少不良反应方面[OR=0.28,95%CI(0.09,0.88),P=0.03]明显优于对照组,但在改善Womac评分[MD=-0.65,95%CI(-3.43,2.13),Z=0.46,P=0.65]方面无明显差异。结论:补肾活血汤可通过抑制白细胞介素-1(IL-1),提高总有效率,降低VAS评分,减少不良反应等方面安全有效治疗KOA,但考虑到本次纳入文献质量及样本量大小,本次结果有待进一步研究。Objective To systematically evaluate the clinical efficacy and safety of Bushen Huoxue Decoction in the treatment of knee osteoarthritis(KOA).Method Manually search related journals based on Chinese and English databases.The search time is self-built until July 2021.After two researchers screened the included literature,extracted the required data,and evaluated the quality of the included literature,the Rev Man5.3 software was used for meta-analysis.Results A total of 10 literatures and 10 studies with 1181 patients were included.Meta-analysis showed that the treatment of KOA in the study group increased the total effective rate[OR=3.80,95%CI(2.53,5.72),Z=6.42,P<0.00001],and decreased the visual analog scale(VAS)[MD=-1.60,95%CI(-2.57,-0.63),P=0.001],inhibition of interleukin-1(IL-1)[SMD=-1.34,95%CI(-2.21,-0.47),P=0.003],In terms of reducing adverse reactions[OR=0.28,95%CI(0.09,0.88),P=0.03]was significantly better than the control group,but it improved the Womac score[MD=-0.65,95%CI(-3.43,2.13),Z=0.46,P=0.65],there is no significant difference.Conclusion Bushen Huoxue Decoction can safely and effectively treat KOA by inhibiting interleukin-1(IL-1),increasing the total effective rate,reducing the VAS score,and reducing adverse reactions.However,considering the quality of the included literature and sample size,The results of this time are for further study.
