摘 要:目的比较基于有效直径(Effective Diameter,De)和水当量直径(Water Equivalent Diameter,D_(w))的体型特异性剂量估算值(Size-Specific Dose Estimate,SSDE)及容积CT剂量指数(Volume CT Dose Index,CTDI_(VOL))在评价婴幼儿心脏CTA检查中辐射剂量的差异并探讨其原因。方法回顾性分析217例患儿心脏CTA检查图像,依据不同检查条件分为3组:A组(0~1)岁、B组(1~3)岁、C组(3~6)岁,计算并分析三组患儿的SSDE及转换因子。结果A组SSDE_(De)及SSDE_(Dw)较CTDI_(VOL)高128.27%(3.04/2.37)、132.49%(3.14/2.37);B组SSDE_(De)及SSDE_(Dw)较CTDI_(VOL)高111.34%(2.65/2.38)、114.29%(2.72/2.38);C组SSDE_(De)及SSDE_(Dw)较CTDI_(VOL)高98.73%(3.10/3.14)、105.41%(3.31/3.14),各分组组内SSDE_(De)较SSDE_(Dw)低估分别为1.85%、1.39%、3.37%。f_(De)及f_(Dw)与De呈高度正相关(R^(2)=0.998、0.802,P<0.01)。CTDI_(VOL)低估了其辐射剂量,直径越小,低估程度越严重。结论SSDE_(Dw)在衡量婴幼儿心脏CTA检查辐射剂量时考虑了婴幼儿检查部位体型及组织的差异,对婴幼儿心脏CTA检查辐射剂量的评价更准确。Objective To explore the differences in radiation doses in infants and young children by comparing body size-specific dose estimates(SSDE)based on effective diameter(De)and water equivalent diameter(Dw)and volume CT dose index(CTDI_(VOL))in cardiac CTA examinations,and to explore the reasons.Methods The cardiac CTA examination images of 217 children were retrospectively analyzed and divided into 3 groups according to different examination conditions:Group A 0 to 1 years old,Group B 1 to 3 years old,and Group C 3 to 6 years old.The size-specific dose estimate(SSDE)values and conversion factors of the three groups were analyzed.Results SSDE of effective diameter(SSDE_(De))and SSDE of water-equivalent diameter(SSDE_(Dw))were 128.27%(3.04/2.37)and 132.49%(3.14/2.37),higher than CTDI_(VOL) in Group A,111.34%(2.65/2.38)and 114.29%(2.72/2.38)higher than CTDI_(VOL) in Group B and 98.73%(3.10/3.14)and 105.41%(3.31/3.14)higher than CTDI_(VOL) in Group C.In three groups,SSDE_(De) were 1.85%,1.39%and 3.37%lower than SSDE_(Dw).The f_(De) and f_(Dw) were highly positively correlated with De(R^(2)=0.998,0.803,P<0.01).CTDI_(VOL) underestimated its radiation dose,and the smaller the body diameter,the more serious the underestimation.Conclusion SSDE_(Dw) takes into account the differences of body type and tissue,so it is more accurate to evaluate the radiation dose of Cardiac CTA in Infants.SSDE_(Dw) considers the differences in body size and tissue of infants and young children when measuring the radiation dose of the cardiac CTA examination,and the evaluation of the radiation dose is more accurate.
