富血小板血浆联合髓芯减压植骨术治疗ARCOⅡ期非创伤性股骨头坏死的效果分析

摘　　要：目的:研用富血小板血浆联合髓芯减压植骨术治疗国际骨微循环研究协会(ARCO)Ⅱ期非创伤性股骨头坏死的效果。方法:本研究选择100例2018年7月至2020年7月期间广水市第一人民医院收治的ARCOⅡ期非创伤性股骨头坏死患者作为研究对象。样本选择完毕后,按照随机数表法将其分为对照组和观察组,每组各有患者50例。用富血小板血浆联合髓芯减压植骨术对观察组患者进行治疗,对照组患者单纯接受髓芯减压植骨术。比较两组患者治疗后的Harris髋关节功能评分(Harris评分)、视觉模拟评分法(VAS)评分及股骨头的存活率。结果:术后,观察组患者Harris评分中的疼痛评分、功能评分、畸形评分、活动范围评分均高于对照组患者,差异有统计学意义(P<0.05)。术后3个月、6个月及12个月,观察组患者的VAS评分均低于对照组患者,差异有统计学意义(P<0.05)。术后对两组患者进行随访(随访至今)得知,观察组患者股骨头的存活率为96.00%,对照组患者股骨头的存活率为84.00%,二者相比差异有统计学意义(P<0.05)。结论:用富血小板血浆联合髓芯减压植骨术治疗ARCOⅡ期非创伤性股骨头坏死的效果显著,能减轻患者术后的疼痛感,改善其髋关节的功能,提高其股骨头的存活率。Objective:To study the effect of platelet-rich plasma combined with core decompression and bone grafting in the treatment of ARCO stageⅱnon-traumatic femoral head necrosis.Methods:A total of 100 patients with ARCO stageⅱnontraumatic femoral head necrosis admitted to The First People’s Hospital of Guangshui city from July 2018 to July 2020 were selected as the study subjects.After sample selection,they were divided into control group and observation group according to random number table method,with 50 patients in each group.Patients in the observation group were treated with platelet-rich plasma combined with core decompression bone grafting,while patients in the control group were treated with core decompression bone grafting alone.Harris hip function score(Harris score),visual analog scale(VAS)score and femoral head survival rate were compared between 2 groups after treatment.Results:After surgery,the pain score,function score,deformity score and range of motion score in the observation group were higher than those in the control group,with statistical significance(P<0.05).VAS scores of patients in the observation group were lower than those in the control group at 3,6 and 12 months after surgery,with statistical significance(P<0.05).Postoperative follow-up of the two groups of patients(follow-up up to now)showed that the femoral head survival rate of the observation group was 96.00%,and that of the control group was 84.00%,the difference was statistically significant(P<0.05).Conclusion:Platelet-rich plasma combined with myeloid core decompression and bone grafting has a significant effect on the treatment of ARCOⅱnon-traumatic femoral head necrosis,which can relieve postoperative pain,improve the function of the hip joint and improve the survival rate of the femoral head.
