血清HDL-C和APO-A检测在冠心病诊断中的应用价值

摘　　要：目的:探讨血清高密度脂蛋白胆固醇(HDL-C)和载脂蛋白A(APO-A)检测在冠状动脉粥样硬化性心脏病(冠心病)诊断中的应用价值。方法:选取宁津县人民医院接诊的冠心病患者32例作为观察组,同时选取在该医院进行体检的36例健康人作为对照组。入组次日采集其静脉血行HDL-C、APO-A检测,比较其检测结果,并采用受试者工作特征(ROC)曲线分析血清HDL-C、APO-A对冠心病的预测价值。结果:观察组患者血清HDL-C、APO-A的水平均低于对照组健康人,差异有统计学意义(P<0.05)。为进一步验证血清HDL-C、APO-A对冠心病的预测价值,采取ROC曲线进行分析,结果显示,血清HDL-C与APO-A联合应用[ROC曲线下面积(AUC)=0.832,P<0.01]对冠心病的预测价值明显优于血清HDL-C(AUC=0.714,P<0.01)或血清APO-A(AUC=0.663,P<0.01)单独应用。结论:冠心病患者血清HDL-C和APO-A的水平均表现出明显下降,二者联合检测在冠心病的诊断中具有较高的应用价值。Objective:To investigate the application value of serum high density lipoprotein cholesterol(HDL-C)and apolipoprotein A(APO-A)in the diagnosis of coronary atherosclerotic heart disease(CORONARY heart disease).Methods:Thirty-two CHD patients treated in Ningjin County People’s Hospital were selected as the observation group,and 36 healthy people were selected as the control group.On the second day after enrollment,venous blood was collected for HDL-C and APO-A detection,and the detection results were compared.The predictive value of serum HDL-C and APO-A on coronary heart disease was analyzed by receiver operating characteristic(ROC)curve.Results:The levels of serum HDL-C and APO-A in observation group were lower than those in control group,the differences were statistically significant(P<0.05).In order to further verify the predictive value of serum HDL-C and APO-A on coronary heart disease,ROC curve analysis was carried out.The results showed that the area under ROC curve(AUC=0.832),The predictive value of P<0.01 was significantly better than that of serum HDL-C(AUC=0.714,P<0.01)or serum APO-A(AUC=0.663,P<0.01)alone.Conclusion:The levels of SERUM HDL-C and APO-A in patients with coronary heart disease showed significant decrease.The combined detection of HDL-C and APO-A has high application value in the diagnosis of coronary h eart disease.
