三步推拿法联合针刺治疗老年腰椎间盘突出症患者的临床观察

摘　　要：目的探讨三步推拿法联合针刺用于老年腰椎间盘突出症(lumbar disc herniation,LDH)患者的价值。方法选取医院2019年10月—2021年9月收治的LDH患者120例为研究对象,以随机数字表法分为观察组与对照组各60例,对照组实施针刺治疗,观察组在对照组基础上加用三步推拿法,比较两组患者的临床疗效、治疗前后疼痛情况及腰椎功能、炎症因子水平、Oswestry功能障碍指数(ODI)评分及不良反应情况。结果治疗后观察组临床有效率显著高于对照组,差异有统计学意义(P<0.05);治疗后两组的日本骨科协会腰椎功能(JOA)评分分数显著上升,视觉模拟评分法(VAS)分数显著下降,观察组的JOA分数显著高于对照组,VAS分数显著低于对照组,差异有统计学意义(P<0.05);治疗后两组的白介素6(IL-6)、肿瘤坏死因子α(TNF-α)水平均显著下降,观察组的IL-6、TNFα水平显著低于对照组,差异有统计学意义(P<0.05);治疗后两组的疼痛、日常活动、单项功能分数均显著降低,观察组的疼痛、日常活动、单项功能分数显著低于对照组,差异有统计学意义(P<0.05);两组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论使用三步推拿法联合针刺治疗老年LDH患者能取得良好疗效,可显著减轻患者疼痛,降低血清炎症因子水平,改善患者腰椎功能。Objective To explore the value of three-step massage combined with acupuncture for elderly patients with lumbar disc herniation(LDH).Methods A total of 120 LDH patients admitted to our hospital from October 2019 to September 2021 were selected as the research objects.They were divided into observation group and control group by random number table method.The control group received acupuncture treatment,and the observation group was added with three-step massage on the basis of the control group.Pain and lumbar function,levels of inflammatory factors,Oswestry Dysfunction Index(ODI)score,and adverse reactions before and after treatment.Results After treatment,the clinical effective rate of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).Significantly decreased,the JOA score of the observation group was significantly higher than that of the control group,and the VAS score was significantly lower than that of the control group,the difference being statistically significant(P<0.05).The levels of TNF-αin the observation group were significantly decreased,and the levels of IL-6 and TNFαin the observation group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).The scores of pain,daily activities and single function in the observation group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Three-step massage combined with acupuncture and moxibustion in the treatment of elderly LDH patients can achieve a good effect,can significantly reduce the pain of patients,reduce the level of serum inflammatory factors,improve the lumbar spine function.
