摘　　要：目的探讨Hoffmann外固定架与内固定分期治疗Schatzker Ⅴ、Ⅵ型胫骨平台骨折的疗效。方法我院就诊的167例胫骨平台骨折患者,按照手术方式分为分期组(Hoffmann外固定架与内固定分期术)和传统组(切开复位内固定术)。比较两组手术时间、术中出血量、住院时间、膝关节评分(HSS)、Lysholm评分、膝关节活动度及并发症发生情况。结果术后与传统组比较,分期组手术和住院时间更短,术后出血量更少,HSS评分优良率更高,屈膝和伸膝最大角度更大,并发症总发生率更低(P<0.05)。结论相比切开复位内固定术治疗Schatzker Ⅴ、Ⅵ型胫骨平台骨折,Hoffmann外固定架与内固定分期治疗手术时间短,手术创伤小,膝关节功能和活动度恢复更佳,术后并发症发生率低,值得临床应用。Objective To explore the curative effect of Hoffmann external fixator and internal fixation staging therapy in the treatment of Schatzker Ⅴ and Ⅵ tibial plateau fractures.Methods A total of 167 patients with tibial plateau fractures treated in our hospital were enrolled. According to different surgical methods, the patients were divided into a staging group and a traditional group. The staging group was treated with Hoffmann external fixator and internal fixation staging therapy while the traditional group was treated with open reduction and internal fixation. Operation time, intraoperative blood loss, hospitalization time, scores of hospital for special surgery knee score(HSS) and Lysholm score, knee range of motion and the occurrence of complications between the two groups were compared.Results Compared to the traditional group, the operation time and hospitalization time were shorter, intraoperative blood loss was less, good rate of HSS score was higher, the maximum angles of knee flexion and extension were greater, and total incidence of complications was lower in the staging group(P<0.05).Conclusion In the treatment of Schatzker Ⅴ and Ⅵ tibial plateau fractures, operation time of the staging therapy is shorter, surgical trauma is less, recovery of knee function and range of motion are better, and incidence of postoperative complications is lower than those of traditional surgical methods. It is worthy of clinical application.
