贫血样本手工推片与自动化推片用于数字化阅片仪的对比研究

摘  要:目的探讨贫血样本自动推片和手工推片的质量控制以及与外周血白细胞(WBC)分类的相关性。方法选取四川省人民医院贫血样本100例,分别进行自动推片和手工推片,并采用数字化细胞形态阅片仪的外周血推片质量分析软件SmearChecker检测其单细胞层长度,采用数字化细胞形态阅片仪采集的红细胞概览图计数单层红细胞(RBC)层不重叠的红细胞数量,并对数字化细胞形态阅片仪白细胞形态分析预分类后再经人工分类审核后的结果,与人工显微镜镜检结果作比较进行线性相关分析。结果与手工推片制备的样本比较,自动推片法制备的样本单细胞层长度更长,合格率更高,红细胞概览图不重叠数量更多(P<0.05),数字化细胞形态阅片仪白细胞分类结果与人工显微镜镜检结果相关性更高(P<0.05)。结论贫血样本在自动推片法制备的外周血推片质量和稳定性更高,有效保证数字化细胞形态阅片仪白细胞分类结果和人工显微镜镜检结果相关性。Objective To explore the quality control of automatic and manual blood smears for anemia samples and their correlation with the classification of peripheral white blood cells(WBC).Methods One hundred blood samples were collected form anemia patients in the Sichuan Provincial People’s Hospital.The automatic and manual blood smears were simultaneously made.The length of the monolayer was detected by using the peripheral blood slide quality analysis software SmearChecker of the digital cell morphology analyzer.The number of red blood cells(RBC)with non-overlapping RBC layers was counted using the RBC overview images collected by the digital cell morphology analyzer.WBC morphology was pre-classified with the morphology analyzer and then reviewed by manual classification.The results were compared with those of manual microscopy by linear correlation analysis.Results Compared with the manual blood smears,the automated blood smears were evaluated with longer monolayer,higher pass rate and more non-overlapped RBC(P<0.05).The results of WBC classification by the digital cell morphology analyzer were more correlated with the results of manual microscopy(P<0.05).Conclusion The quality and stability of the peripheral blood smears prepared by the automatic slide method for anemia samples are higher.The correlation between the WBC classification results of the digital cell morphology analyzer and the results of manual microscopy can be effectively guaranteed.
