止痛如神汤加减对湿热下注型混合痔术后患者的疗效观察

摘　　要：目的探讨止痛如神汤加减在湿热下注型混合痔术后的疗效。方法选取2018年6月至2020年6月本院肛肠外科进行外剥内扎手术的85例混合痔患者,根据治疗方式不同分为研究组45例及对照组40例,研究组采用止痛如神汤加减,对照组采用高锰酸钾溶液坐浴,两组均治疗1周。比较两组患者临床疗效、疼痛视觉模拟量表评分(VAS)、肛缘水肿症状评分、临床指标(创面愈合时间、住院时间、水肿消退时间及疼痛消失时间)、不良反应。结果研究组总有效率95.56%高于对照组的72.50%,差异有统计学意义(P<0.05);治疗后,两组VAS评分均下降,且研究组低于对照组(P<0.05);两组患者1、3、5及7天后疼痛评分及肛缘水肿症状评分均低于治疗前,且研究组低于对照组(P<0.05)研究组创面愈合时间、住院时间、水肿消退时间及疼痛消失时间等明显优于对照组(P<0.05);两组治疗后均未出现不良反应。结论对湿热下注型混合痔术后患者予以止痛如神汤加减治疗具有显著临床疗效,可减轻患者疼痛,改善术后水肿,安全可靠,值得临床推广。Objective To analyze the curative effect of Zhi-tong-ru-shen decoction in the treatment of postoperative patients with damp-heat mixed hemorrhoids.Methods Eighty-five patients with mixed hemorrhoids who underwent external stripping and internal ligation from June 2018 to June 2020 in the anorectal surgery department of our hospital were selected.The patients’data were retrospectively analyzed.According to difference in postoperative treatment methods,they were divided into a study group(n=45)and a control group(n=40).The study group was treated with Zhi-tong-ru-shen decoction,and the control group was treated with potassium permanganate solution sitz bath.Both groups were treated for one week.After treatment,the clinical efficacy,pain visual analogue scale(VAS)scores,symptom scores in edema of anal edge,clinical indicators such as wound healing time,hospital stay,edema recession time and pain disappearance time and adverse reactions were compared between the two groups.Results The total effective rate of the study group was 95.56%that was higher than 72.50%of the control group(P<0.05).After treatment,the VAS scores of the two groups were decreased,and the study group was lower than the control group(P<0.05).The pain scores and symptom scores of anal margin edema of patients after 1,3,5 and 7 days in the two groups were lower than those before treatment,and the pain scores of the study group were lower than those of the control group(P<0.05).The clinical indicators of the study group were significantly better than those of the control group(P<0.05).No serious adverse reactions occurred in the two groups after treatment.Conclusion Zhi-tong-ru-shen decoction has a significant clinical effect in the treatment of postoperative patients with damp-heat mixed hemorrhoids.It can reduce the pain of the patients,and improve the postoperative edema.It is safe and reliable,and worthy of clinical promotion.
