摘 要:目的探究内镜术后不同剂量鼻腔冲洗液对慢性鼻窦炎患者嗅觉功能及血清炎症因子的影响。方法纳入2018年1月至2021年1月我院慢性鼻窦炎患者108例,内镜手术后使用500 ml鼻腔冲洗液的患者设为大剂量组(n=56),内镜手术后使用250 ml鼻腔冲洗液的患者设为小剂量组(n=52),两组患者均治疗3个月,治疗期间持续随访。探究两组患者治疗3个月后临床疗效差异,评估两组患者嗅觉功能、血清炎症因子水平、鼻黏膜纤毛运输功能差异。结果大剂量组患者完全控制率高于小剂量组(P<0.05);治疗1周后,两组患者hs-CRP、IL-4、IL-6均较治疗前下降;治疗3个月后,两组患者识别阈均较治疗前下降,且大剂量组均低于小剂量组(P<0.05);治疗1、3个月后,两组患者鼻腔纤毛传输速率均较治疗前上升,且大剂量组高于小剂量组(P<0.05)。结论大剂量鼻腔冲洗液对慢性鼻窦炎患者鼻内镜手术的临床疗效更好,更有利于患者嗅觉功能恢复,对降低血清炎症因子、促进鼻黏膜纤毛运输功能恢复效果更显著。Objective To explore the effect of nasal irrigation with different doses on olfactory function and serum inflammatory factors in patients with chronic sinusitis after endoscopic surgery.Methods One hundred and eight patients with chronic sinusitis in our hospital between January 2018 and January 2021were selected.Patients using 500 ml nasal irrigation after endoscopic surgery were set as a high-dose group(n=56),and patients using 250 ml nasal irrigation after endoscopic surgery were included in a low-dose group(n=52).Both groups were treated for 3 months and continuously followed up duringthe treatmentperiod.The difference in clinical efficacy between the two groups after 3 months of treatment was explored.The differences in olfactory function,levels of serum inflammatory factors and nasal mucosal ciliary transport function were evaluated in the two groups.Results The complete control rate in the high-dose group was higher than that in the low-dose group(P<0.05).After 1 week of treatment,the levels of high-sensitivity C-reactive protein(hs-CRP),interleukin-4(IL-4)and interleukin-6(IL-6)were declined in the two groups compared with those before treatment.After 3 months of treatment,the identification threshold of the two groups was decreased compared to before treatment,and the above indicators in the high-dose group were lower compared to the low-dose group(P<0.05).After 1 month and 3 months of treatment,the nasalmucosal ciliary transport rate of the two groups was increased compared to before treatment,and the transport rate was higher in the high-dose group than that in the low-dose group(P<0.05).Conclusion High-dose nasal irrigation has a better clinical efficacy in the treatment of patients with chronic sinusitis undergoing endoscopic sinus surgery,conducive to recovery of olfactory function of the patients.Moreover,high-dose is more obvious in reducing the serum inflammatory factors and promoting the recovery of nasal mucosal ciliary transport function.
