摘　　要：目的探讨右美托咪定在妊娠期高血压剖宫产术患者的临床应用及对血清胰岛素样生长因子结合蛋白-1(IGFBP-1)水平的影响及意义。方法选取合并妊娠期高血压择期行剖宫产终止妊娠的产妇80例,采用随机数字表法分为A组和B组各40例。A组于断开脐带后即开始静脉泵注右美托咪定,B组泵注等量生理盐水。所有产妇均采用蛛网膜下腔麻醉,记录麻醉前(T1)及胎盘剥离后3 min(T2)、6 min(T3)、12 min(T4)、20 min(T5)、30 min(T6)时的心率、血压、脉搏血氧饱和度,并测定血清IGFBP-1水平。记录两组腹痛、寒战、恶心、呕吐等不良反应的发生。结果A组产妇心率于T5和T6时显著低于B组(P<0.05);两组间SpO_(2)和MAP比较,差异无统计学意义(P>0.05)。两组T6时血清IGFBP-1水平高于T1时(P<0.05),但组间比较差异无统计学意义(P>0.05)。A组不良反应发生率低于B组(P<0.05)。结论右美托咪定在减少妊娠期高血压产妇剖宫产术的腹痛、寒战、恶心、呕吐等不良反应的同时,对产妇血清IGFBP-1水平无明显影响。Objective To explore the effect and significance of dexmedetomidine on the levels of serum insulin-like growth factor binding protein-1(IGFBP-1)in patients with gestational hypertension undergoing cesarean section.Methods A total of 80 puerperae with gestational hypertension who underwent elective cesarean section to terminate pregnancy were selected.The puerperae were divided into a group A and a group B by random number table method,40 in each group.In the group A,dexmedetomidine was injected intravenously after the umbilical cord was disconnected,and the group B was pumped equal amount of normal saline.All puerperae were under subarachnoid anesthesia.The heart rate,blood pressure,and blood oxygen saturation were recorded before anesthesia(T1)and at 3 minutes(T2),6 minutes(T3),12 minutes(T4),20 minutes(T5),and 30 minutes(T6)after placental abruption.The serum IGFBP-1 levels were also measured Adverse reactions such as abdominal pain,chills,nausea,and vomiting,were recorded.Results The heart rate of puerperae in the group A was significantly lower than that in the group B at T5 and T6(P<0.05).There was no significant difference in blood oxygen saturation(SpO_(2))and mean arterial pressure(MAP)between the two groups(P>0.05).The serum IGFBP-1 levels of the two groups at T6 were higher than those at T1(P<0.05).There was no significant difference in serum IGFBP-1 levels between the two groups at T6(P>0.05).The incidence of adverse reactions in the group B was higher than that in the group A(P<0.05).Conclusion Dexmedetomidine can reduce the adverse reactions such as abdominal pain,chills,nausea,and vomiting during cesarean section in patients with gestational hypertension.It has no significant effect on the levels of serum IGFBP-1.
