摘 要:目的探讨去除污染胚胎透明带阻断污染再次发生的可行性。方法收集2例分别被大肠杆菌和粪肠球菌污染的胚胎培养液,分别感染多原核受精卵(20枚和25枚),之后按照不同的处理方法清除胚胎表面细菌:采用常规培养液洗涤的方法(对照组);胚胎先进行洗涤,之后利用台式酸去除胚胎的透明带(实验组)。观察两组胚胎再次污染发生率、D2胚胎卵裂率、D4胚胎融合率(含部分卵裂球融合)以及囊胚形成率。结果(1)大肠杆菌感染组:对照组在D2全部发生再次污染(10/10),胚胎在D3时均全部发生退化;实验组无一发生再次污染(0/10),D4胚胎融合率为60.00%(6/10),其中1个胚胎未融合的卵裂球与融合部分发生分离,囊胚率为10.00%(1/10)。(2)粪肠球菌感染组:对照组再次污染率为81.82%(9/11),卵裂率为72.73%(8/11),没有胚胎发生融合,无囊胚形成;实验组再次污染率为14.29%(2/14),D4胚胎融合率为50.00%(6/12),其中2枚完全融合、4枚部分融合,没有卵裂球游离现象发生,囊胚率为16.67%(2/12)。结论胚胎发生细菌污染后,去除透明带是一种高效的阻断胚胎再次污染的方法。Objective:To explore the feasibility of removing the zona pellucida of contaminated embryos to prevent the recurrence of contamination.Methods:Two cases of embryo culture medium contaminated by Escherichia coli(E.coli)and Enterococcus faecalis respectively were collected to contaminate 20 and 25 polypronuclear fertilized oocytes respectively.Then,the bacteria on the surface of the embryos were removed by different treatment methods:the embryos were washed with conventional culture medium in the control group,and the embryos were washed firstly,and then the zona pellucida of the embryos were removed with acid Tyrode’s solution in the experimental group.The incidence of recontamination of embryos,the cleavage rate of Day 2 embryos,the fusion rate of Day 4 embryos(including partial blastomere fusion)and the blastocyst formation rate were observed in the two groups.Results:(1)In the E.coli infection group,embryos in the control group were all recontaminated on Day 2(10/10)and degenerated on Day 3.However,no recontamination occurred in the embryos of the experimental group(0/10),the fusion rate of Day 4 embryos was 60.00%(6/10),and the unfused blastomere of one embryo was separated from the fusion part,and the blastocyst rate was 10.00%(1/10).(2)In the Enterococcus faecalis infection group,the recontamination rate of the control group was 81.82%(9/11),and the cleavage rate was 72.73%(8/11).No blastomere fusion occurred,and no blastocyst was formed.In the experimental group,the recontamination rate of embryos was 14.29%(2/14),and the fusion rate of Day 4 embryos was 50.00%(6/12).Among them,2 embryos were completely fused,and 4 embryos were partially fused.No blastomere dissociation occurred,and the blastocyst rate was 16.67%(2/12).Conclusions:After bacterial contamination of embryos,removing the zona pellucida of embryos is an efficient method to prevent embryos recontamination.
