剪切波弹性成像胎盘弹性参数联合正五聚蛋白3、脂肪因子网膜素-1对先兆子痫的诊断价值

摘　　要：目的分析剪切波弹性成像胎盘弹性参数联合正五聚蛋白3(Pentraxin-3)、脂肪因子网膜素-1(Omentin-1)对先兆子痫的诊断价值。方法选择2018年6月-2020年6月在浙江省金华市人民医院就诊的先兆子痫患者90例为研究组,另选同期孕检正常孕妇90例为健康组,比较两组及研究组不同先兆子痫程度中央胎儿面、边缘胎儿面、中央母体面、边缘母体面胎盘弹性参数及血清Pentraxin-3、Omentin-1指标的差异,并绘制ROC曲线,分析胎盘弹性参数联合Pentraxin-3、Omentin-1检测对先兆子痫的诊断价值。结果与健康组相比,研究组胎盘不同区域弹性值较高,Pentraxin-3表达升高,Omentin-1表达降低,差异均有统计学意义(P<0.05);先兆子痫不同程度三组不同区域弹性值及Pentraxin-3、Lumican比较差异均有统计学意义(P<0.01),且随着先兆子痫程度的增加,不同区域弹性值升高、Pentraxin-3升高、Omentin-1降低;联合诊断对先兆子痫的敏感度、特异度和准确度均较高,差异具有统计学差异(P<0.05)。结论剪切波弹性成像胎盘弹性参数及Pentraxin-3、Omentin-1可用于先兆子痫的诊断,且联合使用对先兆子痫的诊断价值更高。Objective To analyze the diagnostic value of shear wave elastography combined with pentaxin-3 and omentin-1 in preeclampsia.Methods 90 patients with preeclampsia treated in Jinhua people's Hospital of Zhejiang Province from June 2018 to June 2020 were selected as the study group,and 90 normal pregnant women were selected as the healthy group.The placental elasticity parameters of central fetal surface,marginal fetal surface,central maternal surface and marginal maternal surface of the two groups and the study group with different degrees of preeclampsia,as well as the differences of serum Pentraxin-3 and omentin-1 indicators were compared,and the ROC curve was drawn,To analyze the diagnostic value of placental elasticity parameters combined with Pentraxin-3 and omentin-1 in preeclampsia.Results compared with the healthy group,the elasticity value of different regions of placenta in the study group was higher,the expression of Pentraxin-3 increased,and the expression of omentin-1 decreased,the difference was statistically significant(P<0.05);There were significant differences in the elastic value of different regions and the comparison of Pentraxin-3 and lumican in the three groups with different degrees of preeclampsia(P<0.01),and with the increase of the degree of preeclampsia,the elastic value of different regions increased,Pentraxin-3 increased,and omentin-1 decreased;The sensitivity,specificity and accuracy of combined diagnosis for preeclampsia were higher,and the difference was statistically significant(P<0.05).Conclusion The parameters of placenta elasticity,Pentraxin-3 and omentin-1 of shear wave elastography can be used in the diagnosis of preeclampsia,and the combined use of them has higher diagnostic value for preeclampsia.
