摘 要:目的探讨保护素DX(PDX)对髋部骨折后急性肺损伤的保护作用及相关机制。方法将30只24月龄的健康老龄雄性SD大鼠随机分为对照组、模型组和PDX干预组,每组10只。对照组和模型组在造模1 h后腹腔注射500μl生理盐水,PDX干预组于造模1 h后腹腔注射500μl浓度为0.002 mg/ml的PDX。所有大鼠于造模24 h后处死并采集相关标本。对各组肺组织进行病理评分,计算各组肺组织湿干质量比,检测各组动脉血氧分压,检测肺泡灌洗液中的总蛋白浓度及相关炎性因子[白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)]的含量,蛋白免疫印迹实验检测肺组织中Toll样受体9(TLR9)、核转录因子-kappaB(NF-κB)蛋白的表达水平,免疫组化实验检测肺组织中性粒细胞的髓过氧化物酶(MPO)。结果三组肺组织湿干质量比、肺泡灌洗液总蛋白、动脉血氧分压、肺损伤评分、TNF-α和IL-6水平、肺组织中TLR9、NF-κB的相对表达水平及组织中MPO比较,差异均有统计学意义(P<0.05)。模型组湿干质量比、肺泡灌洗液总蛋白、肺损伤评分、TNF-α和IL-6水平、肺组织中TLR9、NF-κB的相对表达水平及组织中MPO水平高于对照组,动脉血氧分压低于对照组,差异均有统计学意义(P<0.05)。PDX干预组湿干质量比、肺泡灌洗液总蛋白、肺损伤评分、TNF-α和IL-6水平、肺组织中TLR9、NF-κB的相对表达水平及组织中MPO水平低于模型组,动脉血氧分压高于模型组,差异均有统计学意义(P<0.05)。结论PDX能够明显降低老龄大鼠髋部骨折后的肺损伤程度并明显改善肺功能,这种作用与抑制肺组织中TLR9、NF-κB蛋白的表达和活化,从而抑制中性粒细胞聚集,减轻肺部炎症反应密切相关。Objective To investigate the protective effect of protectin DX(PDX)on acute lung injury after hip fracture and its mechanism in aged rats.Methods Thirty healthy aged male SD rats(24 months)were randomly divided into the control group(n=10),model group(n=10)and PDX group(n=10).At 1 hour after model establishment,the control and model group were intraperitoneally injected with 500μl normal saline,and the PDX group was intraperitoneally injected with 500μl PDX(0.002 mg/mL).All the rats were sacrificed at 24 hours after the establishment of model and the samples were collected.The lung injury score was evaluated by pathological analysis.The ratio of wet weight to dry weight(W/D)of the lung was calculated.The arterial partial pressure of oxygen was measured.Total protein concentration,the levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in alveolar lavage fluid,toll-like receptor 9(TLR9),nuclear transcription factor-kappaB(NF-κB)and myeloperoxidase(MPO)of neutrophils in lung tissue were determined.Results Among the three groups,there were significant differences in lung tissue W/D ratio,total protein concentration in alveolar lavage fluid,arterial partial pressure of oxygen,lung injury score,TNF-αand IL-6 levels,relative expression of TLR9 and NF-κB,and MPO expression in lung tissues(P<0.05).In the model group,lung tissue W/D ratio,total protein concentration in alveolar lavage fluid,lung injury score,TNF-αand IL-6 levels,relative expression of TLR9 and NF-κB,and MPO level in lung tissue were higher than those in the control group;and arterial partial pressure of oxygen was lower than that in the control group,there were significant differences between the two groups(P<0.05).In the PDX group,lung tissue W/D ratio,total protein concentration in alveolar lavage fluid,lung injury score,TNF-αand IL-6 levels,relative expression of TLR9 and NF-κB,and MPO level in lung tissue were lower than those in the model group,and arterial partial pressure of oxygen was higher than that in the model group,there
