老年腹膜透析患者腹膜纤维化与腹膜透析液中糖原合酶激酶-3β、β连环蛋白水平的相关性研究

摘　　要：目的探究老年腹膜透析患者腹膜纤维化与腹膜透析液中糖原合酶激酶-3β(GSK-3β)、β连环蛋白(β-catenin)水平的相关性。方法前瞻性选择2016年1月—2020年12月上海交通大学医学院附属松江医院收治的腹膜透析腹膜纤维化患者81例(观察组)和同期在上海交通大学医学院附属松江医院腹膜透析未发生腹膜纤维化患者100例(对照组)为研究对象,检测并比较2组腹膜透析液中GSK-3β、β-catenin及腹膜纤维化指标[转化生长因子β1(TGF-β1)、α-平滑肌肌动蛋白(α-SMA)、纤维连接蛋白(FN)、I型胶原蛋白(Col-I)]水平,用Spearman检验和Pearson检验进行相关性分析。结果观察组GSK-3β和β-catenin水平高于对照组(P<0.05);观察组TGF-β1、FN、Col-I、α-SMA水平高于对照组(P<0.05);相关性分析提示,腹膜透析液中GSK-3β和β-catenin水平与腹膜透析患者发生腹膜纤维化呈正相关(P<0.05),腹膜透析液中GSK-3β和β-catenin水平均与TGF-β1、FN、Col-I、α-SMA呈正相关(P<0.05)。结论老年腹透患者腹膜纤维化与腹透液中糖原合酶激酶-3β、β连环蛋白水平存在一定相关性(低度),其水平检测对于了解腹膜纤维化发病和病情有一定临床意义。Objective To explore the correlation between peritoneal fibrosis and the levels of glycogen synthase kinase-3β(GSK-3β)andβ-catenin in peritoneal dialysate in elderly patients with peritoneal dialysis.Methods From January2016to December2020,81patients with peritoneal fibrosis(observation group)who underwent peritoneal dialysis in Songjiang Hospital Affiliated to Medical College of Shanghai Jiaotong University and100ones without peritoneal fibrosis(control group)who did peritoneal dialysis in the hospital were prospectively selected as the research subjects.The levels of GSK-3βandβ-catenin and peritoneal fibrosis indexes[transforming growth factorβ1(TGF-β1),α-smooth muscle actin(α-SMA),fibronectin(FN)and type I collagen(CoL-I)]in peritoneal dialysate were detected and compared between the two groups.Spearman test and Pearson test were used for correlation analysis.Results The levels of GSK-3βandβ-catenin in peritoneal dialysate of the observation group was significantly higher than those of the control group(P<0.05),and the levels of TGF-β1,FN,Col-I andα-SMA in peritoneal dialysate of the observation group were significantly higher than those of the control group(P<0.05).Correlation analysis showed that the levels of GSK-3βandβ-catenin in peritoneal dialysate were positively correlated with peritoneal fibrosis in peritoneal dialysis patients(P<0.05),and the levels of GSK-3βandβ-catenin in peritoneal dialysate were positively correlated with TGF-β1,FN,Col-I andα-SMA(P<0.05).Conclusion There is a certain correlation(low grade)between peritoneal fibrosis and the levels of GSK-3βandβ-catenin in peritoneal dialysate in elderly patients with peritoneal dialysis.The detection of their levels has certain clinical significance for understanding the pathogenesis and condition of peritoneal fibrosis.
