摘　　要：目的调查近一年肿瘤专科医院行感染筛查患者梅毒螺旋体血清学阳性率,评价化学发光微粒子免疫分析法(chemiluminescent microparticle immunoassay,CMIA)、梅毒螺旋体颗粒凝集试验(treponema pallidum particle agglutination,TPPA)和梅毒快速血浆反应素环状卡片试验(rapid plasma regain card test,RPR)3种方法检测结果的一致性,为制定合理的梅毒螺旋体血清学筛查和报告模式提供依据。方法梅毒血清学初筛采用CMIA,当S/CO>1时,报告初筛阳性,阴性结果发阴性报告;阳性时,行TPPA和RPR检测。分析不同滴度范围TPPA和RPR的阳性率。结果连续入选40350例行感染筛查患者,初筛阳性者共519例(占1.29%)。初筛阳性者TPPA复检阳性373例(占71.86%),RPR阳性49例(占9.44%)。CMIA检测滴度S/CO在1.1~2.0、2.1~3.0、3.1~4.0、4.1~5.0、5.1~6.0、6.1~10.0、>10时,TPPA与RPR检测的阳性率分别为21%、0%,67%、0%,71%、5%,68%、10%,91%、9%,89%、0%,99%、21%。结论肿瘤专科医院患者梅毒螺旋体血清学特异性抗体CMIA初筛阳性率和TPPA确认试验阳性率与三级综合医院相近,但RPR阳性率较低。初筛滴度边缘升高者,TPPA阴性者占比高。Objective To investigate the positive rate of Treponema Pallidum(TP)serological test screening in an oncology hospital and consistency between Architect Chemiluminescent Microparticle Immunoassay(CMIA)and Treponema Pallidum particle agglutination assay(TPPA).Methods All patients detected with antibody against TP from August,2020 to July,2021 were continuously enrolled for the study.According to the laboratory syphilis confirmatory testing protocol,all samples were initially tested by CMIA and then those samples showed positive result were tested by TPPA and RPR.Results The positive rate of CMIA was 1.29%(519/40345).Three hundred and seventy-three of 519(71.86%)CMIA were TPPA reactive,and 49(9.44%)sera were reactive by RPR.TPPA and RPR positive rates in 7 groups(CMIA S/CO 1.1-2.0,2.1-3.0,3.1-4.0,4.1-5.0,5.1-6.0,6.1-10.0,>10)were 21%/0%,67%/0%,71%/5%,68%/10%,91%/9%,89%/0%,and 99%/21%,respectively.Conclusion Positive rates of Treponema Pallidum serological test(TT)including CMIA and TPPA in an oncology center are similar to those in general hospital,but RPR reactive rate is lower.
