摘 要:目的分析3296例HBsAg阳性母亲所生小儿经疫苗预防后产生HBsAb的保护性效果,为此类小儿预防乙肝提供参考依据。方法HBsAg阳性母亲的新生儿,在出生12h内尽早注射100 IU乙型肝炎免疫球蛋白(hepatitis B immunoglobulin,HBIG),同时在不同部位接种10μg重组酵母乙型肝炎疫苗,并在1月龄和6月龄时分别接种第2针和第3针乙型肝炎疫苗。分析我院2021年1月1日至12月31日期间内HBsAg阳性母亲在我院所生小儿的乙肝病毒标志物(hepatitis B virus marker,HBVM),对0~10岁HBsAb的结果和7~8月的HBVM模式进行回顾性分析。结果HBsAb水平在<10岁的各个年龄段,阴性和低表达(0~100mIU/mL)都存在一定比例,其中9月~10岁比例明显高于7~8月年龄段的比例;7~8月年龄段,具有有效血清学保护作用的HBsAb(>100mIU/mL)占97.5%。7~8月年龄段HBVM除2例表现1235阳免疫失败外,只表现2阳、25阳、245阳3种模式,且HBsAb高表达三者HBVM之间没有显著性差异。结论对HBsAg阳性母亲在我院所生小儿实施疫苗预防后,7~8月就能产生有效保护性HBsAb。HBsAb高表达在不同乙肝五项模式中的分布具有一致性。疫苗预防对HBsAg阳性母亲所生小儿能够起到预防乙肝病毒感染的作用,效果显著。Objective To investigate the production of HBsAb in the immune response of different age groups after the birth of 3296 children of HBV-infected mothers after immune intervention,and to provide references for further prevention of hepatitis B in such children population.Methods We summarized results of our hospital from January 1^(st) 2021 to December 31^(st) 2021 on the hepatitis B virus marker(HBVM)in children with age of 0 to 10 years of HBV-infected mothers during the period and then retrospectively analyzed for HBVM type with age of 7-8 months and HBsAb.Results HBsAb negative and low expression(10-100mIU/mL)in children younger than 10 years were identified in a certain proportion,in which the proportion of 9 months-10 years old was significantly higher than that of 7-8months old,and in 7-8months old HBsAb(>100 mIU/mL)was 97.5%,in 7-8months old HBVM only showed 2positive,25 positive,245 positive type except 2 cases with 1,235 positive immunologic failure.Moreover,the high HBsAb expression was not significantly different between the three types of HBVM.Conclusion The effective protective HBsAb could be produced from 7-8 months old after the children born to HBsAg positive mothers are vaccinated prophylaxis in our hospital.The distribution of high HBsAb expression is consistent among different types of HBVM.The effect of vaccine prevention on Hepatitis B virus infection is significant in infants born to HBsAg-infected mothers.
