股骨近端防旋髓内钉内固定治疗外侧壁骨折的股骨粗隆间骨折过程中重建与未重建外侧壁的疗效比较

摘　　要：目的 分析股骨近端防旋髓内钉内固定治疗外侧壁骨折的股骨粗隆间骨折过程中重建与未重建外侧壁的疗效。方法 60例外侧壁骨折的股骨粗隆间骨折患者,按照奇偶法分为对照组和观察组,每组30例。两组患者均接受股骨近端防旋髓内钉内固定治疗,治疗过程中,对照组患者未重建外侧壁,观察组患者重建外侧壁。比较两组患者临床指标、术前及术后3个月Harris髋关节功能评分、并发症发生情况。结果 观察组患者术中出血量(163.92±44.67)ml少于对照组的(234.16±62.04)ml,卧床时间(10.68±4.63)d、骨折愈合时间(76.34±7.14)d、完全负重时间(116.37±10.45)d短于对照组的(22.67±5.21)、(89.54±11.33)、(131.16±11.27)d,差异具有统计学意义(P<0.05)。术后3个月,两组患者Harris髋关节功能评分均高于本组术前,且观察组患者Harris髋关节功能评分(86.12±7.49)分显著高于对照组的(78.93±6.63)分,差异具有统计学意义(P<0.05)。观察组患者并发症发生率为6.67%,低于对照组的26.67%,差异具有统计学意义(P<0.05)。结论 外侧壁骨折的股骨粗隆间骨折患者接受股骨近端防旋髓内钉内固定治疗效果较好,同时治疗过程中重建外侧壁能够加速患者术后康复,降低术后并发症发生率,改善患者髋关节功能,值得推广应用。Objective To analyze the efficacy of reconstruction and non-reconstruction of the lateral wall during the treatment of femoral intertrochanteric fractures with proximal femoral anti-rotation intramedullary nail internal fixation.Methods A total of 60 cases of femoral intertrochanteric fractures with lateral wall fracture were divided into control group and observation group according to the odd-even method,with 30 cases in each group.Both groups received proximal femoral anti-rotation intramedullary nail internal fixation.During the treatment,the lateral wall was not reconstructed in the control group,while the lateral wall was reconstructed in the observation group.The clinical indicators,Harris hip function score before surgery and 3 months after surgery,and complications were compared between the two groups.Results The intraoperative blood loss(163.92±44.67)ml of the observation group was less than(234.16±62.04)ml of the control group,and the bed time(10.68±4.63)d,fracture healing time(76.34±7.14)d,and the full weight-bearing time(116.37±10.45)d were shorter than(22.67±5.21),(89.54±11.33),and(131.16±11.27)d of the control group.All the differences were statistically significant(P<0.05).At 3 months after surgery,the Harris hip function scores in the two groups were higher than those before surgery in this group,and the Harris hip function score(86.12±7.49)points in the observation group were significantly higher than(78.93±6.63)points in the control group.All the differences were statistically significant(P<0.05).The complication rate in the observation group was 6.67%,which was lower than 26.67%in the control group,and the difference was statistically significant(P<0.05).Conclusion Proximal femoral anti-rotation intramedullary nail internal fixation has good effect in the treatment of remoral intertrochanteric fractures with lateral wall fracture,and reconstruction of the lateral wall during treatment can accelerate postoperative rehabilitation,reduce the postoperative complication rate,and improve t
