倒“V”形切口改良Brisson手术治疗小儿隐匿性阴茎的临床效果研究

摘　　要：目的分析探究小儿隐匿性阴茎患儿接受倒“V”形切口改良Brisson手术治疗的临床疗效。方法160例小儿隐匿性阴茎患儿,按随机分组法分为观察组和对照组,每组80例。观察组行倒“V”形切口改良Brisson手术治疗,对照组行常规Brisson手术治疗。比较两组患儿手术治疗情况(手术时间、包皮水肿消退时间、术后创伤愈合时间、阴茎外观评分)以及术后不同时间的随访情况。结果观察组手术时间(50.03±9.87)min与对照组的(47.84±8.99)min比较,差异无统计学意义(P>0.05)。观察组包皮水肿消退时间(11.09±2.74)d、术后创伤愈合时间(4.02±0.94)d均短于对照组的(27.74±3.88)、(7.18±1.03)d,阴茎外观评分(7.39±1.04)分高于对照组的(4.58±1.01)分,差异均具有统计学意义(P<0.05)。观察组术后1周、2周、1个月、3个月、6个月预后良好率均高于对照组,差异均具有统计学意义(P<0.05)。结论将倒“V”形切口改良Brisson手术治疗方式应用到小儿隐匿性阴茎的临床治疗中,能提升患儿的手术疗效,使患儿在更短时间内达到更好的预后效果,值得推广应用。Objective To analyze and investigate the clinical efficacy of modified Brisson surgery with inverted"V"incision in the treatment of concealed penis in children.Methods A total of 160 children with concealed penis were randomly divided into observation group and control group,with 80 cases in each group.The observation group was treated with modified Brisson surgery with inverted"V"incision,and the control group was treated with conventional Brisson surgery.The surgical treatment(operation time,subsidence time of prepuce edema,postoperative wound healing time,penis appearance score)and follow-up at different time points were compared between the two groups.Results The operation time of the observation group was(50.03±9.87)min,which was not statistically significant compared with(47.84±8.99)min of the control group(P>0.05).The subsidence time of prepuce edema(11.09±2.74)d and postoperative wound healing time(4.02±0.94)d in the observation group were shorter than(27.74±3.88)and(7.18±1.03)d in the control group;the penis appearance score(7.39±1.04)points was higher than(4.58±1.01)points in the control group;all the differences were statistically significant(P<0.05).The good prognosis rates of the observation group were higher than that of the control group at 1 week,2 weeks,1 month,3 months and 6 months after surgery,and the differences were statistically significant(P<0.05).Conclusion The application of modified Brisson surgery with inverted"V"incision to the clinical treatment of concealed penis in children can improve the surgical effect of children and enable children to achieve better prognosis in a shorter time,which is worthy of popularization and application.
