复发性流产患者NKT细胞、DNT细胞、T细胞亚群的变化探讨

摘　　要：目的探讨复发性流产(RSA)患者外周血自然性杀伤T细胞(NKT细胞)、T细胞亚群及双阴性调控性T细胞(DNT细胞)的变化,旨在为复发性流产患者提供参考依据。方法选取265例复发性流产患者(复发性流产组)和145例健康体检妇女(健康对照组)作为研究对象,两组采用流式细胞术检测外周血DNT细胞(CD3^(+)CD4-CD8-)、NKT细胞(CD3^(+)CD56^(+)CD16^(+))及T细胞亚群[CD3^(+)T、CD3^(+)CD4^(+)辅助性T细胞(Th)、CD3^(+)CD8^(+)杀伤T细胞(Tc)及Th/Tc],并进行比较。结果两组DNT细胞比较,差异无统计学意义(P>0.05);复发性流产组NKT细胞水平(9.64±8.97)%高于健康对照组的(3.21±1.51)%,差异具有统计学意义(P<0.05)。两组CD3^(+)T、CD3^(+)CD4^(+)Th、CD3^(+)CD8^(+)Tc及Th/Tc比较,差异均无统计学意义(P>0.05)。结论对复发性流产患者进行外周血NKT细胞的检测有着重要的意义,其既能有效提示临床复发性流产患者存在免疫系统功能异常,又能快速明确病因,对免疫异常者进行有效的干预,可帮助复发性流产患者顺利实现妊娠。Objective To discuss the changes of natural killer T cells(NKT cells),double-negative T cell(DNT cell)and T cells subsets in patients with recurrent spontaneous abortion,so as to provide reference for recurrent spontaneous abortion.Methods A total of 265 patients with recurrent spontaneous abortion(recurrent abortion group)and 145 healthy women(healthy control group)were selected as the research subjects.Peripheral blood DNT cells(CD3^(+)CD4-CD8-),NKT cells(CD3^(+)CD56^(+)CD16^(+))and T cell subsets[CD3^(+)T,CD3^(+)CD4^(+)Helper T cells(Th),CD3^(+)CD8^(+)cytotoxic T cells(Tc)and Th/Tc]were detected by flow cytometry in both groups and compared.Results There was no statistically significant difference in DNT cells between the two groups(P>0.05).The level of NKT cells(9.64±8.97)%in the recurrent abortion group was higher than(3.21±1.51)%in the healthy control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in CD3^(+)T,CD3^(+)CD4^(+)Th,CD3^(+)CD8^(+)Tc and Th/Tc ratio between the two groups(P>0.05).Conclusion The detection of peripheral blood NKT cells in patients with recurrent spontaneous abortion is of great importance,as it can not only effectively indicate the presence of immune system abnormalities in patients,but also quickly identify the cause of the disease and provide effective intervention for those with immune abnormalities,which can help patients with recurrent spontaneous abortion achieve a successful pregnancy.
