硝酸甘油+缬沙坦+低分子肝素钠治疗冠心病心衰的临床效果分析

摘　　要：目的 分析硝酸甘油、缬沙坦、低分子肝素钠联用治疗冠心病心力衰竭(心衰)患者的临床效果及作用机制。方法 116例冠心病心衰患者,采取随机双盲法分为对照组和观察组,各58例。给予对照组硝酸甘油治疗,给予观察组硝酸甘油+缬沙坦+低分子肝素钠治疗。对比两组疗效、治疗前后的临床指标、不良反应发生率。结果 观察组治疗总有效率为98.28%,高于对照组的86.21%,差异有统计学意义(P<0.05)。治疗前,两组6 min步行距离、血浆脑钠肽(BNP)、左心室射血分数(LVEF)比较,差异无统计学意义(P>0.05);治疗后,观察组6 min步行距离(387.29±71.42)m长于对照组的(276.21±86.92)m, BNP(0.31±0.12)μg/L低于对照组的(0.49±0.16)μg/L, LVEF(42.28±8.75)%大于对照组的(36.42±7.83)%,差异有统计学意义(P<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论 酸甘油+缬沙坦+低分子肝素钠治疗冠心病心衰患者疗效显著、不良反应少、可有效改善临床指标,值得推广应用。Objective To analyze the clinical effect and mechanism of nitroglycerin,valsartan and low molecular weight heparin sodium combined treatment in patients with coronary heart disease and heart failure.Methods A total of 116 patients with coronary heart disease and heart failure were randomly divided into control group and observation group by double-blind method,with 58 cases in each group.The control group was treated with nitroglycerin,and the observation group was treated with nitroglycerin+valsartan+low molecular weight heparin sodium.The clinical efficacy,clinical indicators before and after treatment,and the incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 98.28%,which was higher than 86.21%of the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in 6-min walking distance,plasma brain natriuretic peptide(BNP)and left ventricular ejection fraction(LVEF)between the two groups(P>0.05).After treatment,the 6-min walking distance(387.29±71.42)m in the observation group was longer than(276.21±86.92)m in the control group,the BNP(0.31±0.12)μg/L was lower than(0.49±0.16)μg/L in the control group,and the LVEF(42.28±8.75)%was higher than(36.42±7.83)%in the control group.All the differences were statistically significant(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Nitroglycerin combined with valsartan and low molecular weight heparin sodium in the treatment of patients with coronary heart disease and heart failure has a significant effect and less adverse reactions,and can effectively improve clinical indicators,which is worthy of popularization and application.
