宫腔镜联合屈螺酮炔雌醇片治疗子宫内膜息肉的疗效观察

摘　　要：目的研究宫腔镜联合屈螺酮炔雌醇片治疗子宫内膜息肉的临床疗效。方法80例子宫内膜息肉患者,按照随机分组法分为观察组和对照组,每组40例。对照组采用宫腔镜切除手术治疗,观察组在对照组基础上联合屈螺酮炔雌醇片治疗。对比两组患者的子宫内膜厚度、月经恢复情况、复发率以及生活质量评分,统计观察组患者不良反应发生率。结果观察组患者的子宫内膜厚度为(1.07±0.31)cm、复发率为5.00%(2/40),均优于对照组的(1.33±0.42)cm、20.00%(8/40),差异具有统计学意义(t/χ^(2)=3.150、4.114,P=0.002、0.043<0.05)。观察组患者的月经恢复率为90.00%,高于对照组的67.50%,差异具有统计学意义(P<0.05)。观察组患者社会、情感以及健康评分分别为(8.57±2.63)、(8.49±2.71)、(8.56±2.34)分,均高于对照组的(6.45±1.89)、(6.37±2.02)、(6.53±1.86)分,差异具有统计学意义(t=4.140、3.967、4.295,P=0.000、0.000、0.000<0.05)。观察组患者用药后出现恶心1例、情绪波动异常1例,不良反应发生率为5.00%(2/40)。患者不良反应均自行缓解。结论宫腔镜联合屈螺酮炔雌醇片治疗子宫内膜息肉具有明显的临床疗效,能够显著改善患者的子宫内膜厚度、月经恢复情况以及生活质量,降低患者的复发率,不良反应少,值得临床研究和应用。Objective To study the clinical efficacy of hysteroscopy combined with drospironone and ethinylestradiol tablets in the treatment of endometrial polyps.Methods A total of 80 patients with endometrial polyps were randomly divided into observation group and control group,with 40 cases in each group.The control group was treated with hysteroscopic resection,and the observation group was treated with drospirenone and ethinylestradiol tablets on the basis of the control group.The endometrial thickness,menstrual recovery,recurrence rate,and quality of life scores were compared between the two groups,and the incidence of adverse reactions in the observation group was counted.Results The endometrial thickness of the observation group was(1.07±0.31)cm,and the recurrence rate was 5.00%(2/40),which were better than(1.33±0.42)cm and 20.00%(8/40)of the control group,and the differences were statistically significant(t/χ^(2)=3.150,4.114;P=0.002,0.043<0.05).The menstrual recovery rate of the observation group was 90.00%,which was higher than 67.50%of the control group,and the difference was statistically significant(P<0.05).The social,emotional and health scores in the observation group were(8.57±2.63),(8.49±2.71),and(8.56±2.34)points,which were higher than(6.45±1.89),(6.37±2.02),and(6.53±1.86)points in the control group,and the differences were statistically significant(t=4.140,3.967,4.295;P=0.000,0.000,0.000<0.05).The observation group had 1 case of nausea and 1 case of abnormal mood swing occurred after the medication,with an incidence of adverse reactions of 5.00%(2/40).The adverse reactions of the patients all resolved spontaneously.Conclusion Hysteroscopy combined with drospironone and ethinylestradiol tablets has better clinical efficacy in the treatment of endometrial polyps,and can significantly improve the endometrial thickness,menstrual recovery and quality of life of patients,reduce the recurrence rate and incidence of adverse reactions of patients,which is worthy of clinical research and application.
