摘　　要：目的观察引流阀(FP7)植入术与小梁切除联合羊膜移植术对难治性青光眼的降眼压作用。方法回顾性病例研究。收集我科收治的28例(30只眼)难治性青光眼患者临床资料,按照手术方式不同分为两组,A组行Ahmed FP7型引流阀植入术,B组行经典小梁切除联合羊膜移植术。观察两组术前、术后眼压值、并发症及成功率等相关情况。结果两组术后眼压较术前均有明显下降;术后1个月、6个月两组之间差异无统计学意义(P>0.05);术后1年,A组眼压值明显低于B组,差异有统计学意义(P<0.05)。到末次随访,A组成功率高于B组,并发症低于B组。结论青光眼引流阀FP7植入术治疗难治性青光眼较小梁切除联合羊膜移植术效果显著。Objective To observe the IOP lowering effect of drainage valve(FP7)implantation and trabeculectomy combined with amniotic membrane transplantation on refractory glaucoma.Methods A total of 28 patients(30 eyes)with refractory glaucoma admitted to our department were retrospectively divided into two groups according to different surgical methods:group A underwent Ahmed FP7 drainage valve implantation,and group B underwent classical trabeculectomy combined with amniotic membrane transplantation.Preoperative and postoperative intraocular pressure,complications and success rate of the two groups were observed.Results The intraocular pressure in both groups decreased significantly after operation.There was no significant difference between the two groups 1 month and 6 months after surgery(P>0.05).One year after surgery,iOP in group A was significantly lower than that in group B,the difference was statistically significant(P<0.05).By the last follow-up,the composition power of A was higher than that of B,and the complications were lower than that of B.Conclusions Compared with trabeculectomy combined with amniotic membrane transplantation,FP7 implantation of glaucoma drainage valve is more effective in treating refractory glaucoma.
